The causes of IBS are still largely unknown, but preliminary research points to different genetic, hormonal &amp; dietary factors that may play a role. Read on to learn the details about each one.<!--more--><\/p>\n<h2><span id=\"What_is_Irritable_Bowel_Syndrome\"><b>What is Irritable Bowel Syndrome?<\/b><\/span><\/h2>\n<p>The NIH estimates that as many as 20% of Americans may experience signs of IBS, but doctors still don\u2019t know the exact causes behind this syndrome (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Linaclotide\">R<\/a>).<\/p>\n<p>As you will see, IBS is a complex disease and there are many potential variables in the web of causes.<\/p>\n<p>There are two main types of IBS &#8211; one that involves Diarrhea (IBS-D) and one that involves Constipation (IBS-C).<\/p>\n<p>Food goes from stomach -&gt; small intestine -&gt; large intestine.<\/p>\n<p>The large intestine is loaded with bacteria, but the rest of the gastrointestinal tract has relatively few bacteria under normal conditions. When a person has bacteria in the small intestine, unabsorbed carbohydrates are fermented there, producing significant amounts of gas.<\/p>\n<p>This can cause cramping and burning because the gas stretches the small intestine. The gas can also push the intestinal content rapidly toward your large intestine to cause diarrhea. Bacteria generally ferment in the small intestines when gut \u2018flow&#8217; (motility\/peristalsis) is slowed down in this area [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2890937\/\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5347643\/\">R<\/a>].<\/p>\n<p>When people have diarrhea, it usually means the flow is quickened in the large intestine.<\/p>\n<p>Gut pain comes from the neurons in the gut being \u2018hypersensitive\u2019. When the intestines expand, this is sensed by the neurons in the gut. If neurons are hypersensitive, this distension will feel uncomfortable or hurt.<\/p>\n<p>This post is mainly about IBS, but other gut problems may stem from the same causes.<\/p>\n<h2><span id=\"Potential_Contributing_Factors\">Potential Contributing Factors<\/span><\/h2>\n<p><i>None of the below factors are established causes of IBS. According to preliminary research, they may play a role in this complex syndrome, but the evidence is inconclusive. If you recognize one or more of these factors in your case, it doesn\u2019t mean you have IBS. If you\u2019re experiencing digestive issues, work with your doctor to get adequate diagnosis and treatment.<\/i><\/p>\n<h3><span id=\"1_Stress\"><b>1) Stress<\/b><\/span><\/h3>\n<p>Chronic stress response activation (HPA) is believed to be a contributing factor of anxiety disorders, bipolar, insomnia, PTSD, borderline personality, ADHD, major depression, burnout, chronic fatigue syndrome, fibromyalgia, IBS, alcoholism, and other diseases (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Hypothalamic%E2%80%93pituitary%E2%80%93adrenal_axis\">R<\/a>).<\/p>\n<p>Chronic stress is probably the most common and significant cause of gut flow (motility) changes. Indeed, increased activation of the stress pathway (<a href=\"https:\/\/selfhacked.com\/blog\/reasons-cortisol-low-high\/\">HPA axis<\/a>) is associated with IBS (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2095678\/\">R<\/a>).<\/p>\n<h4><b>CRH<\/b><\/h4>\n<p>The effects of stress on the gut mostly have to do with the effects of<a href=\"https:\/\/selfhacked.com\/blog\/why-your-health-problems-worsen-after-stress-the-role-of-crh\/\"> Corticotropin-Releasing Hormone<\/a> (CRH), which is released from the<a href=\"https:\/\/selfhacked.com\/blog\/hypothalamus-101\/\"> hypothalamus<\/a> when stressed (<a href=\"http:\/\/www.sciencedirect.com\/science\/article\/pii\/0196978186900641\">R<\/a>).<\/p>\n<p>CRH may play a role in IBS by increasing the flow or motor speed in the colon and decreasing it in the small intestine. When the flow is decreased in the small intestine, bad bacteria overgrow by feeding on carbs and produce gas (<a href=\"http:\/\/gut.bmj.com\/content\/42\/6\/845.full\">R<\/a>).<\/p>\n<p>This fermentation in the small intestine can increase methane-producing bacteria from carbs, which produces methane gas (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3388522\/#bibr8-1756283X11436241\">R<\/a>,<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3388522\/#bibr11-1756283X11436241\"> R<\/a>).<\/p>\n<p>People who get IBS are more susceptible to gut dysfunction when CRH is released. In IBS, there are increased HPA axis responses to stressors such as meals, hormones and mental stress compared to controls (<a href=\"http:\/\/gut.bmj.com\/content\/42\/6\/845.full\">R<\/a>, <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3761093\/#B127\">R<\/a>).<\/p>\n<p>Therefore, stress is more harmful to people with IBS with regard to gut function.<\/p>\n<p>CRH also causes the gut to be \u2018hypersensitive\u2019 and experience pain more. When given a drug that blocks CRH, some patients experienced pain relief (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17238026\">R<\/a>).<\/p>\n<p>CRH may also play a role by:<\/p>\n<ul>\n<li>Increasing inflammation (<a href=\"https:\/\/selfhacked.com\/blog\/supplements-foods-exercise-right-type-th1-vs-th2-dominance\/\">Th1 dominance<\/a>, <a href=\"https:\/\/selfhacked.com\/blog\/nuclear-factor-kappa-beta\/\">Nf-kB<\/a>, <a href=\"https:\/\/selfhacked.com\/blog\/interleukin-1\/\">IL-1b<\/a>, <a href=\"https:\/\/selfhacked.com\/blog\/interleukin-6\/\">IL-6<\/a>, <a href=\"https:\/\/selfhacked.com\/blog\/supplements-lifestyle-factors-influence-tnf-interleukin-6-il-6\/\">TNF<\/a> (13X), MHC-II (HLA-DR), TLR-4 and ICAM-1 expression) (<a href=\"http:\/\/www.nature.com\/jid\/journal\/v127\/n3\/full\/5700607a.html\">R<\/a>, <a href=\"http:\/\/www.endocrine-abstracts.org\/ea\/0013\/ea0013s27.htm\">R<\/a>).<\/li>\n<li>Mediating autoimmunity. In rheumatoid arthritis, Hashimoto\u2019s and UC, inflamed tissues contained large amounts of CRH (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9629233\">R<\/a>). In a mouse model of arthritis, blocking CRH reduced inflammation and markers of cartilage destruction (<a href=\"http:\/\/www.researchgate.net\/publication\/11309393_Corticotropin_releasing_hormone_%28CRH%29_antagonist_attenuates_adjuvant_induced_arthritis_role_of_CRH_in_peripheral_inflammation\">R<\/a>).<\/li>\n<li>Activating mast cells and therefore <a href=\"https:\/\/selfhacked.com\/blog\/deal-histamine\/\">histamine issues<\/a>, which explains in part why stress induces allergic symptoms (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9629233\">R<\/a>).<\/li>\n<\/ul>\n<p>Stress will also cause the release of various neurotransmitters (catecholamines), which decreases intestinal flow (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3079864\/\">R<\/a>).<\/p>\n<p>Chronic stress or frequent activation of the HPA axis can cause <a href=\"https:\/\/selfhacked.com\/blog\/oxidative-stress-101\/\">oxidative stress<\/a> as well (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23490070\">R<\/a>).<\/p>\n<p>Check out the posts on <a href=\"https:\/\/selfhacked.com\/blog\/stress-bad-uncovering-hidden-effects-stress\/\">why stress is bad<\/a> and <a href=\"https:\/\/selfhacked.com\/blog\/reasons-cortisol-low-high\/\">all of the triggers of stress response<\/a> for more details.<\/p>\n<h4><b>Adrenaline<\/b><\/h4>\n<p>Adrenaline and <a href=\"https:\/\/selfhacked.com\/blog\/norepinephrine-stress-hormone\/\">noradrenaline<\/a>, through ADRA2A receptors, may inhibit gut flow (<a href=\"https:\/\/en.wikipedia.org\/wiki\/Alpha-2_adrenergic_receptor\">R<\/a>).<\/p>\n<p>In addition, they cause a short-lived increase in blood pressure, but a long term decrease in blood pressure. They also cause sedation and thicker blood (platelet aggregation) (<a href=\"https:\/\/en.wikipedia.org\/wiki\/Alpha-2_adrenergic_receptor\">R<\/a>).<\/p>\n<p>These can be common symptoms in people with IBS-C.<\/p>\n<h3><span id=\"2_Inflammation\"><b>2) Inflammation<\/b><\/span><\/h3>\n<p>In some sense, IBS may be considered IBD-lite, which means it\u2019s like <a href=\"https:\/\/selfhacked.com\/blog\/ibd-ulcerative-colitis-crohn-genetics-lifestyle-stress-infections\/\">IBD<\/a> (Crohn&#8217;s, colitis), but with lower levels of inflammation. Patients with IBD experience IBS-like symptoms when their IBD cools down (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Irritable_bowel_syndrome\">R<\/a>).<\/p>\n<p>A three-year study found that patients diagnosed with IBS were 16.3 times more likely to be diagnosed with IBD during the study period (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Irritable_bowel_syndrome\">R<\/a>).<\/p>\n<p>The risk of developing IBS increases sixfold after acute gut infection. After salmonella, the risk can increase by 8X. This may be because infections cause inflammation and<a href=\"https:\/\/selfhacked.com\/blog\/oxidative-stress-101\/\"> oxidative stress<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24780949\">R<\/a>, <a href=\"http:\/\/onlinelibrary.wiley.com\/doi\/10.1111\/j.1365-2036.2007.03399.x\/abstract\">R<\/a>).<\/p>\n<p>Inflammation can affect the gut in many ways. One way is by activating our stress response\/HPA axis (<a href=\"http:\/\/www.nutritionandmetabolism.com\/content\/7\/1\/79\">R<\/a>).<\/p>\n<p>People with IBS may have immune reactions to dietary proteins. In one study, food elimination based on IgG antibodies resulted in a significant decrease in symptoms of IBS (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16762076\">R<\/a>).<\/p>\n<p>Mast cells may increase in intestinal mucosa in patients with IBS, especially by intestinal nerves. According to limited research, they can directly influence gut flow and result in an increase in abdominal pain and discomfort (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16762076\">R<\/a>).<\/p>\n<p>There\u2019s evidence that anti-inflammatory drugs such as Mesalazine help treat IBS. Mesalazine helps to normalize the gut flora and reduce gut permeability (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Irritable_bowel_syndrome#Research\">R<\/a>).<\/p>\n<p>A wide variety of inflammation types can cause IBS. There\u2019s no one cytokine that\u2019s dominant.<\/p>\n<p>According to some authors, people with IBS are more like to have:<\/p>\n<ul>\n<li><a href=\"https:\/\/selfhacked.com\/blog\/supplements-people-th1-dominant\/\">Th1 Dominance<\/a> &#8211; people with IBS are more likely to be Th1 dominant (elevated IL-12) (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24780949\">R<\/a>). Interferon reduces serotonin in the gut and also increases <a href=\"https:\/\/selfhacked.com\/blog\/oxidative-stress-101\/\">oxidative stress<\/a> (by activating IDO) (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>). However, people with Th2 can also have IBS because of Mast cells and other types of inflammation.<\/li>\n<li><a href=\"https:\/\/selfhacked.com\/blog\/th17\/\">Th17 Dominance<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20058616\">R<\/a>)<\/li>\n<li><a href=\"https:\/\/selfhacked.com\/blog\/supplements-foods-exercise-right-type-th1-vs-th2-dominance\/\">Th2 Dominance<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3761093\/#B127\">R<\/a>) &#8211; Th2 cytokines quicken gut flow. Stimulated IL-5 and IL-13 are higher in IBS.<\/li>\n<li><a href=\"https:\/\/selfhacked.com\/blog\/supplements-lifestyle-factors-influence-tnf-interleukin-6-il-6\/\">Higher TNF<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17383420\">R<\/a>, <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16279907\">R<\/a>) especially IBS-D (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3761093\/table\/T2\/\">R<\/a>)<\/li>\n<li><a href=\"https:\/\/selfhacked.com\/blog\/interleukin-1\/\">Higher IL-1b<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17383420\">R<\/a>)<\/li>\n<li><a href=\"https:\/\/selfhacked.com\/blog\/interleukin-6\/\">Higher IL-6<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22740130\">R<\/a>, <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17383420\">R<\/a>) &#8211; capable of stimulating gut neurons (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3761093\/#B127\">R<\/a>)<\/li>\n<li>Higher <a href=\"https:\/\/selfhacked.com\/blog\/c-reactive-protein\/\">CRP<\/a> &#8211; People with IBS have an average hs-CRP of 1.17, while healthy controls have a level of 0.72 (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21951717\">R<\/a>). Reuteri increased gut motility and decreases pain perception (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19210574\">R<\/a>).<\/p>\n<p>Novel research suggests microbial infections as important contributors to IBS [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4721968\/\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4051916\/\">R<\/a>].<\/p>\n<h3><span id=\"6_Antibiotic_Usage\"><b>6) Antibiotic Usage<\/b><\/span><\/h3>\n<p>Antibiotics may disturb the gut microbiome by killing good bacteria; their use is among the potential risk factors for developing IBS [<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24486051\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/30189148\">R<\/a>].<\/p>\n<p>However, some antibiotics like rifaximin may have treatment potential [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/27737852\">R<\/a>].<\/p>\n<p><b>Further research should cast more light on the connection between the microbiome, antibiotics, and IBS.<\/b><\/p>\n<h3><span id=\"7_Intestinal_Permeability\"><b>7) Intestinal Permeability<\/b><\/span><\/h3>\n<p>Intestinal permeability plays a role in a range of health disorders, including IBS and IBD [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5988153\/\">R<\/a>, <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2095678\/\">R<\/a>].<\/p>\n<p>Stress can increase gut permeability. In animal models, both acute and chronic stress enhance the ability of bacteria to adhere and penetrate the gut, which causes gut inflammation [<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2095678\/\">R<\/a>].<\/p>\n<p>Hypersensitivity from stress results from an alteration of colon permeability [<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2095678\/\">R<\/a>].<\/p>\n<p>According to preliminary research, probiotics such as <a href=\"https:\/\/selfhacked.com\/blog\/health-benefits-l-plantarum\/\">L. plantarum<\/a> [<a href=\"http:\/\/en.wikipedia.org\/wiki\/Lactobacillus_plantarum#Therapeutics\">R<\/a>], <a href=\"https:\/\/selfhacked.com\/blog\/l-rhamnosus\/\">L. rhamnosus<\/a>, <a href=\"https:\/\/selfhacked.com\/blog\/b-longum\/\">B. infantis<\/a>, and <a href=\"https:\/\/selfhacked.com\/blog\/s-boulardii\/\">S. boulardii<\/a> may reduce gut permeability and inflammation [<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2095678\/\">R<\/a>].<\/p>\n<p>Lectins and gluten might also increase gut permeability in sensitive individuals [<a href=\"https:\/\/pdfs.semanticscholar.org\/04e9\/81e8dc20d608029d44da6b460f94e37d8ced.pdf\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3384703\/\">R<\/a>]. Elimination diets such as the <a href=\"https:\/\/selfhacked.com\/blog\/products\/lectin-avoidance-diet-cookbook\/\">Lectin Avoidance Diet<\/a> may help identify food irritants,<b> but their therapeutic value remains unknown<\/b>.<\/p>\n<h3><span id=\"8_FODMAPs\"><b>8) FODMAPs<\/b><\/span><\/h3>\n<p><a href=\"https:\/\/selfhacked.com\/blog\/fodmap-food-list\/\">FODMAP<\/a><a href=\"https:\/\/selfhacked.com\/blog\/fodmap-food-list\/\">s<\/a> are types of fiber that result in fermentation and associated gas production, which can cause gut distension and bloating.<\/p>\n<p>Preliminary research shows some IBS improvement when people go on a <a href=\"https:\/\/selfhacked.com\/blog\/fodmap-diet\/\">low FODMAP diet<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24076059\">R<\/a>).<\/p>\n<h3><span id=\"9_Serotonin\"><b>9) Serotonin<\/b><\/span><\/h3>\n<p>Most of the serotonin in the body (95%) is found in the gut (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>).<\/p>\n<p><a href=\"https:\/\/selfhacked.com\/blog\/serotonin-happy-neurotransmitter\/\">Serotonin<\/a> quickens gut flow by speeding up motor neurons and releases chloride. In one study, serotonin cells were significantly higher in IBS-D and lower in IBS-C than in controls (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22057239\">R<\/a>).<\/p>\n<p>This makes sense. If there\u2019s less serotonin, constipation will occur and if there\u2019s too much, diarrhea will occur.<\/p>\n<p>Serotonin transport is also reduced in some patients with IBS, possibly because of interferon-gamma (<a href=\"https:\/\/selfhacked.com\/blog\/supplements-people-th1-dominant\/\">Th1<\/a>) (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/#CIT0016\">R<\/a>).<\/p>\n<h3><span id=\"10_Excess_IDO_Low_Tryptophan\"><b>10) Excess IDO, Low Tryptophan<\/b><\/span><\/h3>\n<p>The enzyme IDO1 degrades tryptophan into kynurenine and is needed to create <a href=\"https:\/\/selfhacked.com\/blog\/treg\/\">Tregs<\/a> (Good) (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21517752\">R<\/a>).<\/p>\n<p>However, IDO1 may sustain self-attacking antibody production by B cells (Bad), and contribute to chronic inflammation (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21517752\">R<\/a>).<\/p>\n<p>Some researchers think too much IDO may contribute to IBS. The conversion of tryptophan to kynurenine causes too much <a href=\"https:\/\/selfhacked.com\/blog\/oxidative-stress-101\/\">oxidative stress<\/a> in the gut and too little serotonin (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>).<\/p>\n<p>High kynurenine and low tryptophan is a potential marker for IBS, but this imbalance is found in other inflammatory conditions, so it\u2019s not specific (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>).<\/p>\n<p>Inflammation from interferon gamma (Th1 cytokine) or LPS from bacterial infections will increase IDO (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20872026\/\">R<\/a>).<\/p>\n<p>Some studies have found that interferon gamma increases in the small intestine after consuming gluten (by gluten-sensitive patients without celiac disease) (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>).<\/p>\n<h4><b>IBS And The Gut Immune System:<\/b><\/h4>\n<p>AhR is a receptor in gut cells that helps the development of tolerance to proteins and also curbs the growth of bad bacteria (candida) by producing antimicrobial peptides. It regulates the number of Tregs, cytotoxic T cells, B cells and mast cells (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/#CIT0036\">R<\/a>).<\/p>\n<p>Too little activation of this gut immune system may result in autoimmunity and allergies (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/#CIT0036\">R<\/a>).<\/p>\n<p>Good bacteria (LAB such as <a href=\"https:\/\/selfhacked.com\/blog\/top-12-scientific-health-benefits-of-l-reuteri\/\">L Reuteri<\/a>) eat tryptophan as a source of energy and produce molecules that activate AhR (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>). Kynurenine also activates AhR (<a href=\"http:\/\/journal.frontiersin.org\/Journal\/10.3389\/fimmu.2014.00640\/full\">R<\/a>).<\/p>\n<p>This may curb the growth of Candida and boosts the growth of good bacteria (<a href=\"https:\/\/selfhacked.com\/blog\/the-brains-secret-backup-generator\/\">lactic acid<\/a> bacteria) (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>).<\/p>\n<p>Without tryptophan in their &#8216;diet&#8217;, microbes do not produce molecules that activate AhR (IAld). This results in less good bacteria (lactobacilli) and more bad bacteria (Candida), which may be a marker of IBS (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>).<\/p>\n<p>However, it&#8217;s a double-edged sword because tryptophan metabolism will cause oxidative stress (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>).<\/p>\n<p>Cruciferous veggies contain indoles, which activate AhR and may contribute to a favorable microbial environment in the stomach (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>).<\/p>\n<p>A variety of herbal extracts such as <a href=\"https:\/\/selfhacked.com\/blog\/ginseng\/\">ginseng<\/a>, <a href=\"https:\/\/selfhacked.com\/blog\/licorice\/\">licorice<\/a>, Gingko Biloba may cause AhR activation, but the clinical significance of this effect is not clear (<a href=\"http:\/\/journal.frontiersin.org\/Journal\/10.3389\/fimmu.2014.00640\/full\">R<\/a>).<\/p>\n<p>AhR activation is not always positive, as it can activate mast cells (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4266036\/\">R<\/a>).<\/p>\n<h3><span id=\"11_TGR5_and_Bile\"><b>11) TGR5 and Bile<\/b><\/span><\/h3>\n<p>TGR5 is a receptor that lines our gut and is activated by <a href=\"https:\/\/selfhacked.com\/blog\/bile-supplements\/\">bile<\/a> acids and some herbs (<a href=\"http:\/\/www.sciencedirect.com\/science\/article\/pii\/S1590865813006579\">R<\/a>).<\/p>\n<p>It enhances energy expenditure, increases oxygen consumption, prevents <a href=\"https:\/\/selfhacked.com\/blog\/132-biological-mechanisms-for-weight-modulation\/\">obesity<\/a>, and decreases insulin resistance in a mice model of obesity. In humans, it converts <a href=\"https:\/\/selfhacked.com\/blog\/thyroid-hormones-t4t3\/\">T4<\/a> into <a href=\"https:\/\/selfhacked.com\/blog\/thyroid-hormones-t4t3\/\">T3<\/a> (<a href=\"http:\/\/www.sciencedirect.com\/science\/article\/pii\/S1590865813006579\">R<\/a>).<\/p>\n<p>TGR5 and its activation by bile acids speed up the gut flow (<a href=\"http:\/\/www.sciencedirect.com\/science\/article\/pii\/S1590865813006579\">R<\/a>).<\/p>\n<p>Mice without TGR5 have leaky gut (<a href=\"http:\/\/www.sciencedirect.com\/science\/article\/pii\/S1590865813006579\">R<\/a>).<\/p>\n<p>Therefore, a deficiency of bile could lead to IBS-C and too much bile could lead to IBS-D. (See below for variations in the TGR5 receptor) (<a href=\"http:\/\/www.sciencedirect.com\/science\/article\/pii\/S1590865813006579\">R<\/a>).<\/p>\n<p>Indeed, up to 30% of people with IBS-D have bile acid malabsorption (which is a different condition, but the results are similar: diarrhea). Some of them do well with bile acid sequestrants (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19570102\">R<\/a>).<\/p>\n<p><b>Still, the science behind this connection is young and requires further confirmation.<\/b><\/p>\n<h3><span id=\"12_Low_Stomach_Acidity\"><b>12) Low Stomach Acidity<\/b><\/span><\/h3>\n<p>Stomach acidity prevents the growth of bacteria. If the stomach isn\u2019t acidic enough, more bacteria will reach the small intestine.<\/p>\n<p>A meta-analysis found that proton pump inhibitors, which suppress stomach acid, can contribute to <a href=\"https:\/\/selfhacked.com\/blog\/sibo-causes-and-treatment\/\">small intestinal bacterial overgrowth (SIBO)<\/a> (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/28770351\">R<\/a>).<\/p>\n<h2><span id=\"IBS_and_Hormones\"><b>IBS and Hormones<\/b><\/span><\/h2>\n<p><i>The section below discusses possible connections between different hormones and IBS, based on preliminary research. If your lab results for any hormone are irregular, make sure to discuss them with your doctor.<\/i><\/p>\n<h3><span id=\"13_Excess_CCK\"><b>13) Excess CCK<\/b><\/span><\/h3>\n<p><a href=\"https:\/\/selfhacked.com\/blog\/good-bad-effects-cck\/\">CCK<\/a> may contribute to gas production after meals. This might be because of activation of the <a href=\"https:\/\/selfhacked.com\/blog\/28-ways-to-stimulate-your-vagus-nerve-and-all-you-need-to-know-about-it\/\">vagus nerve<\/a> or because CCK directly interacts with the hypothalamus to stimulate the colon (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11025366\">R<\/a>).<\/p>\n<h3><span id=\"14_Excess_Cortisol\"><b>14) Excess Cortisol<\/b><\/span><\/h3>\n<p>Cortisol is unusually high in some women with IBS (<a href=\"https:\/\/selfhacked.com\/blog\/app\/uploads\/2017\/12\/IBS-and-Hormones.pdf\">R<\/a>).<\/p>\n<p>CRH may cause significantly more <a href=\"https:\/\/selfhacked.com\/blog\/acth\/\">ACTH<\/a> and cortisol to be released in people with IBS when compared to people without IBS (<a href=\"http:\/\/gut.bmj.com\/content\/42\/6\/845.full\">R<\/a>).<\/p>\n<p>When researchers evaluated the psychological stress in one study, they found that higher cortisol levels in IBS could not be explained by differences in psychological stress (<a href=\"https:\/\/www.med.unc.edu\/ibs\/files\/2017\/10\/IBS-and-Hormones.pdf\">R<\/a>).<\/p>\n<h3><span id=\"15_Sex_Hormones_Testosterone_Estrogen_Progesterone\"><b>15) Sex Hormones: Testosterone, Estrogen, Progesterone<\/b><\/span><\/h3>\n<p>According to limited clinical evidence, low <a href=\"https:\/\/selfhacked.com\/blog\/estradiol\/\">estrogen<\/a> and <a href=\"https:\/\/selfhacked.com\/blog\/top-21-science-based-health-benefits-progesterone-4-drawbacks\/\">progesterone<\/a> may play a role in IBS. Their receptors are found in the gut (<a href=\"https:\/\/www.med.unc.edu\/ibs\/files\/2017\/10\/IBS-and-Hormones.pdf\">R<\/a>).<\/p>\n<p>Some women are more likely to experience symptoms such as stomach pain, diarrhea, nausea, and bloating when estrogen and progesterone drop down to the lowest levels in the body (during the menses) (<a href=\"https:\/\/www.med.unc.edu\/ibs\/files\/2017\/10\/IBS-and-Hormones.pdf\">R<\/a>).<\/p>\n<p>During the low estrogen and progesterone phase (menses), some women with IBS are more sensitive to gut pain. Bloating also seems to be worse during the phase associated with higher progesterone (the luteal phase) (<a href=\"https:\/\/www.med.unc.edu\/ibs\/files\/2017\/10\/IBS-and-Hormones.pdf\">R<\/a>).<\/p>\n<h3><span id=\"16_Decreased_Motilin\"><b>16) Decreased Motilin<\/b><\/span><\/h3>\n<p><a href=\"https:\/\/selfhacked.com\/blog\/what-is-motilin-function-its-role-in-gut-health\/\">Motilin<\/a> is a hormone that stimulates the gut flow and is normally decreased after meals but increased in between meals or <a href=\"https:\/\/selfhacked.com\/blog\/intermittent-fasting\/\">fasting<\/a>. Insulin inhibits motilin (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Motilin\">R<\/a>).<\/p>\n<p>Some IBS patients have decreased motilin response to both a meal and water. However, they show an increase in motilin in response to mental stress and this causes abnormally increased gut activity (<a href=\"https:\/\/www.med.unc.edu\/ibs\/files\/2017\/10\/IBS-and-Hormones.pdf\">R<\/a>).<\/p>\n<h3><span id=\"17_Excess_VIP\"><b>17) Excess VIP<\/b><\/span><\/h3>\n<p><a href=\"https:\/\/selfhacked.com\/blog\/vip-a-potent-anti-inflammatory-hormone-and-natural-ways-to-increase-it\/\">VIP<\/a> stimulates the intestines. It stimulates the secretion of water and electrolytes, bicarbonate and pepsin (an enzyme) and inhibits gastric acid secretion (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Vasoactive_intestinal_peptide\">R<\/a>).<\/p>\n<p>Thus, it may contribute to cramps and watery diarrhea when highly concentrated in the gut (<a href=\"https:\/\/selfhacked.com\/blog\/app\/uploads\/2017\/12\/IBS-and-Hormones.pdf\">R<\/a>).<\/p>\n<p>In two studies, VIP was twice as concentrated in IBS patients compared to control subjects (<a href=\"https:\/\/www.med.unc.edu\/ibs\/files\/2017\/10\/IBS-and-Hormones.pdf\">R<\/a>).<\/p>\n<p>In another study, VIP was only high in women with IBS-D (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24256141\">R<\/a>).<\/p>\n<h3><span id=\"18_Low_PYY\"><b>18) Low PYY<\/b><\/span><\/h3>\n<p>PYY stimulates the absorption of water and electrolytes and inhibits PGE2 and VIP, which stimulates intestinal fluid secretion (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22057239\">R<\/a>).<\/p>\n<p>It acts to slow down the small intestines. According to preliminary research, PYY cells are reduced in both IBS-D and IBS-C (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22057239\">R<\/a>).<\/p>\n<h3><span id=\"19_Ghrelin\"><b>19) Ghrelin<\/b><\/span><\/h3>\n<p>In one trial, <a href=\"https:\/\/selfhacked.com\/blog\/ghrelin-role-cognitive-gut-function-weight-inflammation-autoimmunity\/\">ghrelin<\/a> cell density was significantly higher in IBS-D and lower in IBS-C than in the controls (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24891930\">R<\/a>).<\/p>\n<h3><span id=\"20_Secretin_GIP\"><b>20) Secretin, GIP<\/b><\/span><\/h3>\n<p>In some IBS-diarrhea patients, there are fewer cells that release CCK, secretin, GIP, and somatostatin (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20300845\">R<\/a>).<\/p>\n<p>These hormones stimulate bicarbonate, enzyme secretion, and gall bladder contraction (CCK) (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20300845\">R<\/a>).<\/p>\n<h3><span id=\"21_Other\"><b>21) Other<\/b><\/span><\/h3>\n<p>According to limited clinical evidence, irregular levels of the following hormones may also be found in IBS patients:<\/p>\n<ul>\n<li>Oxyntomodulin (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24316398\">R<\/a>)<\/li>\n<li>Somatostatin (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24891930\">R<\/a>)<\/li>\n<li>Substance P (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24256141\">R<\/a>)<\/li>\n<li><a href=\"https:\/\/selfhacked.com\/blog\/trh\/\">TRH<\/a> (<a href=\"http:\/\/europepmc.org\/backend\/ptpmcrender.fcgi?accid=PMC1867720&amp;blobtype=pdf\">R<\/a>)<\/li>\n<\/ul>\n<p><b>Once again, the role of the above hormones in IBS is not well established. The findings presented here should drive further research but shouldn\u2019t be used to make any health recommendations.<\/b><\/p>\n<h2><span id=\"IBS_and_Other_Factors\">IBS and Other Factors<\/span><\/h2>\n<h3><span id=\"22_Defective_Mucus_Layer\"><b>22) Defective Mucus Layer<\/b><\/span><\/h3>\n<p>The gut mucus layer is an important barrier to keep out the microbes. If it\u2019s not functioning properly (not enough mucin), bacteria may cross over, cause inflammation and disturb the <a href=\"https:\/\/selfhacked.com\/blog\/how-your-gut-microbiota-can-make-you-fat-or-thin\/\">gut microbiota<\/a>. These changes are common in IBS patients (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3887012\/\">R<\/a>, <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3740207\/\">R<\/a>).<\/p>\n<h3><span id=\"23_Guanylate_cyclase_2C\"><b>23) Guanylate cyclase 2C<\/b><\/span><\/h3>\n<p>Guanylate cyclase 2C is a receptor in your gut cells that influences gut flow. When it\u2019s activated, it speeds up the flow.<\/p>\n<p>A drug called Linaclotide uses this mechanism (FDA approved in 2012 for IBS-C). It reduces the activation of sensory neurons in the colon, potentially reducing pain. At the same time, it activates colonic motor neurons, which increases gut flow and thus promotes bowel movements (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3638410\/\">R<\/a>).<\/p>\n<h3><span id=\"24_Nutrient_Deficiencies\"><b>24) Nutrient Deficiencies<\/b><\/span><\/h3>\n<p><a href=\"https:\/\/selfhacked.com\/blog\/35proven-health-benefits-vitamin-d-part-1\/\">Vitamin D<\/a> deficiency may contribute to IBS and some case reports show improvement in symptoms (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23239770\">R<\/a>).<\/p>\n<p>A low intake of <a href=\"https:\/\/selfhacked.com\/blog\/vitamin-b6\/\">vitamin B6<\/a> may correlate with worse symptoms of IBS (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21636013\">R<\/a>).<\/p>\n<p><b>The research is far from conclusive.
