A review of six clinical trials (711 patients) suggested its ability to shield the nerves against toxic<a href=\"https:\/\/selfhacked.com\/blog\/glucose\/\"> glucose<\/a> levels and reduce pain in people with diabetes. However, the results are less convincing when it comes to nerve conduction speed [<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1878764917300037\">R<\/a>].<\/p>\n<h4><b>Anti-HIV Treatment<\/b><\/h4>\n<p>Anti-HIV drugs can also cause nerve damage, <i>antiretroviral toxic neuropathy (ATN)<\/i>, which is a major drawback of HIV treatment. Long-term treatment with acetyl-L-carnitine (1.5-3g daily for 3+ months) was safe and significantly improved the patients\u2019 mental condition [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12598816\">R<\/a>].<\/p>\n<p>Alzheimer\u2019s patients receiving ALCAR had improved [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12598816\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/2178869\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/2201659\"> R<\/a>]:<\/p>\n<ul>\n<li>Attention and memory<\/li>\n<li>Mental agility and focus (reaction time)<\/li>\n<li>Speech and behavior<\/li>\n<\/ul>\n<p>Another large review by came to a similar conclusion, though the authors emphasized most improvements were subjective. They also pointed to other statistical limitations and concluded that<b> more research is needed to confirm the clinical effectiveness of ALCAR<\/b> [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12804452\">R<\/a>].<\/p>\n<p>According to a study of 23 patients with drug-resistant Alzheimer&#8217;s disease, acetyl-carnitine may boost the effectiveness of standard drugs, such as donepezil (Aricept) and rivastigmine (Exelon) [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12841930\">R<\/a>].<\/p>\n<div class=\"sh-summary\"><div class=\"sh-summary-content\">Acetyl-L-carnitine may help with Alzheimer\u2019s disease by boosting acetylcholine and reducing oxidative stress in the brain. It may also improve the effectiveness of drug treatment.<\/div><\/div>\n<h3><span id=\"4_Fertility_and_Sexual_Function\"><b>4) Fertility and Sexual Function<\/b><\/span><\/h3>\n<h4><b>Men<\/b><\/h4>\n<p>In a meta-analysis of nine clinical trials with over 800 infertile men, a combination of L-carnitine and acetyl-L-carnitine (ALCAR) significantly improved sperm mobility. As a result, the chance of a successful pregnancy increased more than<b> 4 times<\/b> [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17392136\">R<\/a>].<\/p>\n<p>In 120 older patients, a combination of acetyl-carnitine and propionyl-carnitine (2 g daily each, for 6 months) was as effective as testosterone in improving sexual dysfunction. Carnitines showed even better effects on erectile function and, unlike testosterone, didn\u2019t cause prostate enlargement [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15072869\">R<\/a>].<\/p>\n<p><i>Peyronie&#8217;s disease<\/i> manifests with a curved penis during erection, due to the presence of scar tissue or plaque, which may lead to erectile dysfunction. In a study of 48 patients with this condition, ALCAR showed better results than a drug, tamoxifen.
