Read on to find out.<!--more--><\/p>\n<h2><span id=\"Executive_Summary\"><b>Executive Summary<\/b><\/span><\/h2>\n<ul>\n<li>There\u2019s evidence against the concept of &#8220;adrenal fatigue&#8221;.<\/li>\n<li>The main symptoms of &#8220;adrenal fatigue&#8221; are most likely caused by other scientifically-valid concepts.<\/li>\n<li>Inflammation and<a href=\"https:\/\/selfhacked.com\/blog\/oxidative-stress-101\/\"> oxidative stress<\/a> are the fundamental causes of what people call &#8220;adrenal fatigue&#8221;.<\/li>\n<li>The<a href=\"https:\/\/selfhacked.com\/blog\/hypothalamus-101\/\"> hypothalamus<\/a> &#8220;senses&#8221; inflammation and OS and sends out &#8220;signals&#8221; to the body (via <a href=\"http:\/\/selfhacked.com\/blog\/how-to-increase-orexin-and-decrease-fatigue-naturally\/\">orexin<\/a>, neurotransmitters, and other hormones) to behave in a way that makes us feel tired, sick, unmotivated, etc.<\/li>\n<li>The active thyroid hormone (<a href=\"https:\/\/selfhacked.com\/blog\/thyroid-hormones-t4t3\/\">T3<\/a>) is decreased by inflammation and OS, which independently causes problems of its own.<\/li>\n<li>The <a href=\"https:\/\/selfhacked.com\/blog\/people-get-tired-even-enough-sleep\/\">tiredness<\/a> probably comes from the hypothalamus (suppression of orexin) and from <a href=\"http:\/\/selfhacked.com\/blog\/low-thyroid-hormones-low-t3-syndrome\/\">low T3<\/a> and low <a href=\"http:\/\/selfhacked.com\/blog\/top-11-scientific-health-benefits-pregnenolone-including-drawbacks\/\">pregnenolone<\/a>.<\/li>\n<li>In some cases, low <a href=\"https:\/\/selfhacked.com\/blog\/need-know-cortisol-health-effects\/\">cortisol<\/a> can also contribute to tiredness, and cortisol levels are also controlled by the hypothalamus. Chronic inflammation generally increases cortisol levels.<\/li>\n<li>Generally, there is no defect or problem with the adrenal glands themselves &#8211; in other words, they are not &#8220;fatigued&#8221;.<\/li>\n<\/ul>\n<p>To address so-called &#8220;adrenal fatigue&#8221;, one would need to find out what factors are causing inflammation and oxidative stress.<\/p>\n<p><a href=\"https:\/\/selfhacked.com\/blog\/the-cause-of-brain-fog\/\">Read this post to learn about another potential source of &#8220;adrenal fatigue&#8221;.<\/a><\/p>\n<h2><span id=\"What_is_Adrenal_Fatigue\"><b>What is \u201cAdrenal Fatigue\u201d?<\/b><\/span><\/h2>\n<p>James Wilson coined the term \u201cadrenal fatigue\u201d in his 1998 book of the same name.<\/p>\n<p>The adrenals produce several hormones such as adrenaline, which is responsible for the \u201cfight-or-flight\u201d response.<\/p>\n<p>So-called &#8220;adrenal fatigue&#8221; is the belief that the adrenal glands have become exhausted and unable to produce adequate quantities of hormones, primarily cortisol and adrenaline.<\/p>\n<h3><span id=\"Is_There_Any_Scientific_Support_For_Adrenal_Fatigue\">Is There Any Scientific Support For \u201cAdrenal Fatigue\u201d?<\/span><\/h3>\n<p>A search of PubMed for<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed?term=%22adrenal%20fatigue%22\"> \u201cadrenal fatigue\u201d<\/a> brings up only eight results.<\/p>\n<p>None of the results contain any substantive information or provide any markers or a sound biological basis for adrenal fatigue.<\/p>\n<p>On the other hand, <a href=\"https:\/\/selfhacked.com\/blog\/chronic-fatigue-syndrome-real-heres-test\/\">chronic fatigue syndrome<\/a>, for example, has plenty of biological markers to speak of and scientific studies behind it.<\/p>\n<p>The problem with the adrenal fatigue concept is that the <b>underlying biology seems false<\/b> since our adrenals have a pretty good capacity to pump out enough cortisol.<\/p>\n<p>When animals undergo chronic stress, their <b>adrenals get bigger. <\/b>The body has a good ability to adapt and make more cortisol (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16772325\">R<\/a>).<\/p>\n<p>Adrenal fatigue <b>shouldn\u2019t be confused with Addison\u2019s disease<\/b> or adrenal insufficiency &#8211; an insufficient production of adrenal hormones due to an underlying condition. The symptoms of Addison\u2019s disease include [<a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/addisons-disease\/expert-answers\/adrenal-fatigue\/faq-20057906\">R<\/a>]:<\/p>\n<ul>\n<li>Fatigue<\/li>\n<li>Body aches<\/li>\n<li>Low blood pressure<\/li>\n<li>Unexplained weight loss<\/li>\n<li>Loss of body hair<\/li>\n<li>Lightheadedness<\/li>\n<\/ul>\n<p><b>If you notice any of the above symptoms, make sure to see your doctor and get adequate medical care. Addison\u2019s disease can have serious complications if left untreated.<\/b><\/p>\n<p>Nonspecific symptoms of fatigue aren\u2019t evidence of adrenal fatigue because there are many scientifically validated causes of fatigue that don\u2019t arise from adrenal dysfunction.<\/p>\n<p>Low or high cortisol levels are found in many conditions, especially cognitive disorders.<\/p>\n<p>Chronic <a href=\"https:\/\/selfhacked.com\/blog\/factors-counteract-stress-response\/\">stress response<\/a> activation contributes to a variety of health problems, including<a href=\"https:\/\/selfhacked.com\/blog\/natural-treatments-depression\/\"> depression<\/a>. However, the adrenals themselves are not the problem. Inflammation and other factors seem to play a central role.<\/p>\n<h3><span id=\"What_Endocrinologists_Have_to_Say_About_Adrenal_Fatigue\">What Endocrinologists Have to Say About \u201cAdrenal Fatigue\u201d<\/span><\/h3>\n<p>The public education arm of the Endocrine Society, representing 14,000 endocrinologists, recently issued the following advisory:<\/p>\n<p>\u201c<i>&#8216;Adrenal fatigue&#8217; is not a real medical condition. There are no scientific facts to support the theory that long-term mental, emotional, or physical stress drains the adrenal glands and causes many common symptoms<\/i>.&#8221; (<a href=\"http:\/\/www.hormone.org\/public\/myths_facts.cfm\">R<\/a>)<\/p>\n<h2><span id=\"The_SymptomsCriteria_of_Adrenal_Fatigue\"><b>The Symptoms\/Criteria of \u201cAdrenal Fatigue\u201d<\/b><\/span><\/h2>\n<p>According to adrenalfatigue.org, answers to the following questions can help uncover \u201cadrenal fatigue\u201d:<\/p>\n<ol>\n<li>Tired for no reason?<\/li>\n<li>Having trouble getting up in the morning?<\/li>\n<li>Need coffee, cola, salty or sweet snacks to keep going?<\/li>\n<li>Feeling run down and stressed?<\/li>\n<li>Crave salty or sweet snacks?<\/li>\n<li>Struggling to keep up with life\u2019s daily demands?<\/li>\n<li>Can\u2019t bounce back from stress or illness?<\/li>\n<li>Not having fun anymore?<\/li>\n<li>Decreased sex drive?<\/li>\n<\/ol>\n<p>If you answered yes to any of these questions, <b>you may have adrenal fatigue<\/b>.<\/p>\n<p>Additional symptoms and criteria include:<\/p>\n<ul>\n<li>One or more severely stressful events that have affected well-being<\/li>\n<li>Hopelessness and despair<\/li>\n<li>Trouble thinking clearly or finishing your tasks<\/li>\n<li>Unexplained fears\/anxieties<\/li>\n<li>Low blood pressure<\/li>\n<li>Frequently <a href=\"http:\/\/selfhacked.com\/blog\/12-reasons-embrace-cold\/\">cold<\/a><\/li>\n<li>Often become hungry, confused, shaky<\/li>\n<li>Hands and legs get restless<\/li>\n<li>Lost<a href=\"https:\/\/selfhacked.com\/blog\/a-comprehensive-list-of-effective-ways-to-lose-weight\/\"> weight<\/a> without reason while feeling very tired and listless<\/li>\n<\/ul>\n<p>The problem is that almost anyone could be diagnosed with \u201cadrenal fatigue\u201d using the above criteria.<\/p>\n<h3><span id=\"How_is_Adrenal_Fatigue_Diagnosed\">How is Adrenal Fatigue Diagnosed?<\/span><\/h3>\n<p>Anecdotally, it\u2019s diagnosed by checking cortisol levels in the morning and throughout the day. This is highly problematic as a diagnostic test because <b>many different factors can influence cortisol levels<\/b>.<\/p>\n<h2><span id=\"Numerous_Potential_Causes_of_Fatigue\"><b>Numerous Potential Causes of Fatigue<\/b><\/span><\/h2>\n<p>Fatigue is a general symptom that can be caused by numerous causes.<\/p>\n<p>All of the symptoms and diagnostic criteria above can be explained in a way that has much more evidence to it.<\/p>\n<p>First, the <b>hypothalamus plays a major role in fatigue;<\/b> it works principally through orexin, but also histamine neurons.<\/p>\n<p>In short, the most common cause of fatigue may be inflammation (<a href=\"http:\/\/selfhacked.com\/blog\/interleukin-1\/\">IL-1<\/a>,<a href=\"https:\/\/selfhacked.com\/blog\/supplements-lifestyle-factors-influence-tnf-interleukin-6-il-6\/\"> TNF<\/a>) and mitochondrial dysfunction, both of which lead to the suppression of orexin neurons.<\/p>\n<p>This inflammation can be localized in the hypothalamus. Inflammation directly leads to the suppression of orexin.<\/p>\n<p>Mitochondrial dysfunction indirectly leads to the suppression of orexin by reducing orexin sensitivity, which is mediated by high glucose (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22005675\">R<\/a>).<\/p>\n<p>Since Dr. Wilson coined the term \u201cadrenal fatigue\u201d, science discovered mitochondrial dysfunction and the fact that inflammation suppresses orexin.<\/p>\n<h2><span id=\"Is_Salivary_Cortisol_a_Good_Test\"><b>Is Salivary Cortisol a Good Test?<\/b><\/span><\/h2>\n<p>Cortisol levels may be a good indicator of someone\u2019s <a href=\"http:\/\/selfhacked.com\/blog\/what-it-means-to-keep-to-a-circadian-rhythm\/\">circadian rhythm<\/a>.<\/p>\n<p>When orexin neurons are suppressed, that interferes with cortisol production, as Orexin A stimulates cortisol release (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11158046\">R<\/a>).<\/p>\n<p>Since orexin is being suppressed (from mitochondrial dysfunction and inflammation), cortisol rhythms often shift and people have lower AM cortisol and higher relative PM cortisol. This is one<b> possible\/theoretical reason<\/b>.<\/p>\n<p>When cortisol is low, it\u2019s not feeding back to inhibit <a href=\"http:\/\/selfhacked.com\/blog\/why-your-health-problems-worsen-after-stress-the-role-of-crh\/\">CRH<\/a>, which is causing more hypothalamic dysregulation as a result of acutely excess CRH.<\/p>\n<p>On the other hand, if cortisol is high, that could mean that CRH is chronically high.<\/p>\n<p>Countless studies speak about<a href=\"https:\/\/selfhacked.com\/blog\/reasons-cortisol-low-high\/\"> cortisol being too low or high<\/a> in relation to various diseases, usually cognitive-based conditions. For example, <a href=\"https:\/\/selfhacked.com\/blog\/ptsd-causes-treatments\/\">PTSD<\/a> has been associated with lower cortisol and higher <a href=\"http:\/\/selfhacked.com\/blog\/dhea-dhea-s\/\">DHEA<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2829297\/\">R<\/a>).<\/p>\n<p>The question is: \u201cwhy?\u201d<\/p>\n<h2><span id=\"Why_Cortisol_is_Low_and_DHEA_is_High\"><b>Why Cortisol is Low and DHEA is High<\/b><\/span><\/h2>\n<p>It\u2019s unlikely that cortisol is low from adrenal malfunction because DHEA is also produced by the adrenal gland and it\u2019s relatively high usually.<\/p>\n<p>One reason why this imbalance may occur is because of the hormone CRH (the release hormone for <a href=\"https:\/\/selfhacked.com\/blog\/acth\/\">ACTH<\/a> and Cortisol).<\/p>\n<p>CRH directly stimulates DHEA production in human adrenal cells. CRH increased the enzymes that shunt pregnenolone production to DHEA, which deprives cortisol production and causes low cortisol (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10523022\">R<\/a>).<\/p>\n<p>The enzyme that causes cortisol to increase (3betaHSD via PKA) is stimulated by ACTH (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10523022\">R<\/a>), but ACTH may be reduced in chronic stress as a result of <a href=\"http:\/\/selfhacked.com\/blog\/30-scientifically-proven-health-benefits-glutathione\/\">glutathione<\/a> depletion from<a href=\"https:\/\/selfhacked.com\/blog\/oxidative-stress-101\/\"> oxidative stress<\/a> caused by CRH (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/25649650\">R<\/a>) (just a theory).<\/p>\n<p>Low cortisol levels could also be due to<a href=\"https:\/\/selfhacked.com\/blog\/reasons-cortisol-low-high\/\"> HPA axis<\/a> under-activity, increased negative feedback sensitivity (from cortisol) and\/or changes in enzymes that metabolize it.<\/p>\n<p>In PTSD, people are more sensitive to cortisol inhibiting the HPA axis because of higher glucocorticoid receptor density (<a href=\"http:\/\/press.endocrine.org\/doi\/full\/10.1210\/en.2011-1218\">R<\/a>).<\/p>\n<p>There are also changes in the enzymes 11\u03b2-HSD1, 11\u03b2-HSD2, and 5\u03b1-reductase, which affect cortisol levels (<a href=\"http:\/\/press.endocrine.org\/doi\/full\/10.1210\/en.2011-1218\">R<\/a>).<\/p>\n<h2><span id=\"Whats_Really_Going_on_in_Adrenal_Fatigue\"><b>What\u2019s Really Going on in \u201cAdrenal Fatigue\u201d?<\/b><\/span><\/h2>\n<p>The following are possible explanations for the most common symptoms attributed to \u201cadrenal fatigue\u201d:<\/p>\n<p>1) Fatigue. Already talked about. The lateral hypothalamus controls fatigue via orexin. This has nothing to do with cortisol levels.<\/p>\n<p>2) Stress. It may actually be more from CRH (produced in the hypothalamus) than cortisol.<a href=\"https:\/\/selfhacked.com\/blog\/reasons-cortisol-low-high\/\"> Stress response<\/a> can activate for a number of reasons.<\/p>\n<p>A variety of cytokines like<a href=\"https:\/\/selfhacked.com\/blog\/interleukin-1\/\"> IL-1<\/a>,<a href=\"https:\/\/selfhacked.com\/blog\/supplements-lifestyle-factors-influence-tnf-interleukin-6-il-6\/\"> TNF<\/a> and <a href=\"http:\/\/selfhacked.com\/blog\/interleukin-6\/\">IL-6<\/a> can stimulate the hypothalamus to release CRH, which eventually increases cortisol.<\/p>\n<p>3) Cravings. The ventromedial hypothalamus (VMH) is actually the part of the brain that is responsible for cravings and appetite. It works via<a href=\"https:\/\/selfhacked.com\/blog\/how-to-increase-orexin-and-decrease-fatigue-naturally\/\"> orexin<\/a>,<a href=\"https:\/\/selfhacked.com\/blog\/npy\/\"> NPY<\/a>, <a href=\"http:\/\/selfhacked.com\/blog\/all-about-leptin-its-role-in-chronic-inflammation-cfs-and-weight\/\">Leptin<\/a> and other hormones.<\/p>\n<p>4) Low mood and <a href=\"http:\/\/selfhacked.com\/blog\/21-drugs-increase-motivation\/\">motivation<\/a>. Motivation is based on epinephrine and<a href=\"https:\/\/selfhacked.com\/blog\/dopamine\/\"> dopamine<\/a> in the hypothalamus (<a href=\"http:\/\/www.medpagetoday.com\/Pediatrics\/ADHD-ADD\/15872\">R<\/a>), as well as orexin and <a href=\"https:\/\/selfhacked.com\/blog\/mch-blood-test-high-low-levels-normal-range\/\">MCH<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24750426\">R<\/a>). Dopamine can activate orexin (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15978010\">R<\/a>).<a href=\"https:\/\/selfhacked.com\/blog\/how-to-increase-orexin-and-decrease-fatigue-naturally\/\"> Read how orexin affects our mood and motivation<\/a>.<\/p>\n<p>5) Gut problems. The hypothalamus controls two chemicals called VIP and CRH, both of which influence gut function. CRH has a known role in<a href=\"https:\/\/selfhacked.com\/blog\/causes-ibs-general-gut-dysfunction\/\"> IBS<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17238026\">R<\/a>). Various cytokines like <a href=\"http:\/\/selfhacked.com\/blog\/il-8\/\">IL-8<\/a>, IL-6, <a href=\"http:\/\/selfhacked.com\/blog\/interleukin-1\/\">IL-1b<\/a>, and TNF also contribute to IBS and gut problems (directly and indirectly). TNF and inflammation increase CRH (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/2161738\">R<\/a>).<a href=\"https:\/\/selfhacked.com\/blog\/oxidative-stress-101\/\"> Oxidative stress<\/a> and inflammation may be at the root of gut problems, not low cortisol levels.<\/p>\n<p>6) Cold hands and feet. The paraventricular nucleus and ventromedial hypothalamus (VMH) controls temperature regulation (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Ventromedial_nucleus#cite_note-15\">R<\/a>). In cases of inflammation and elevated <a href=\"http:\/\/selfhacked.com\/blog\/supplements-lifestyle-factors-influence-tnf-interleukin-6-il-6\/\">TNF-alpha<\/a>, this suppresses orexin and therefore appetite and causes fatigue. Orexin suppression also causes lower body temperature (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11393177\">R<\/a>) and therefore cold.<\/p>\n<p>7) Decreased\/increased appetite. The hypothalamus control appetite through orexin,<a href=\"https:\/\/selfhacked.com\/blog\/ghrelin-role-cognitive-gut-function-weight-inflammation-autoimmunity\/\"> ghrelin<\/a>,<a href=\"https:\/\/selfhacked.com\/blog\/npy\/\"> NPY<\/a>, T3,<a href=\"https:\/\/selfhacked.com\/blog\/all-about-leptin-its-role-in-chronic-inflammation-cfs-and-weight\/\"> leptin<\/a>,<a href=\"https:\/\/selfhacked.com\/blog\/norepinephrine-stress-hormone\/\"> norepinephrine<\/a>, <a href=\"https:\/\/selfhacked.com\/blog\/how-to-increase-serotonin\">serotonin<\/a>, MCH, <a href=\"http:\/\/selfdecode.com\/gene\/fgf2\/?utm_source=seo&amp;utm_medium=selfhacked&amp;utm_campaign=id00002\">FGF2<\/a><a href=\"https:\/\/selfdecode.com\/gene\/fgf21\/?utm_source=seo&amp;utm_medium=selfhacked&amp;utm_campaign=id00002\">1<\/a>+19, and<a href=\"https:\/\/selfhacked.com\/blog\/glp-1-a-hormone-that-causes-weight-loss-and-good-cognitive-effects\/\"> GLP-1<\/a> all of which influence appetite and interact with each other (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Neuropeptide_Y\">R<\/a>,<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12023064\"> R<\/a>,<a href=\"http:\/\/en.wikipedia.org\/wiki\/Leptin#Central_.28hypothalamic.29\"> R<\/a>,<a href=\"http:\/\/en.wikipedia.org\/wiki\/Orexin\"> R<\/a>).<\/p>\n<p>Orexin, T3, ghrelin, MCH, FGF21, and<a href=\"https:\/\/selfhacked.com\/blog\/npy\/\"> NPY<\/a> increase appetite, while leptin,<a href=\"https:\/\/selfhacked.com\/blog\/insulin-101\/\"> insulin<\/a>, norepinephrine, serotonin,<a href=\"https:\/\/selfhacked.com\/blog\/glp-1-a-hormone-that-causes-weight-loss-and-good-cognitive-effects\/\"> GLP-1<\/a>, and <a href=\"https:\/\/selfdecode.com\/gene\/fgf19\/?utm_source=seo&amp;utm_medium=selfhacked&amp;utm_campaign=id00002\">FGF19<\/a> are appetite suppressants. Orexin is activated by<a href=\"https:\/\/selfhacked.com\/blog\/glutamate\/\"> glutamate<\/a> and suppressed by <a href=\"https:\/\/selfhacked.com\/blog\/natural-ways-to-increase-gaba\/\">GABA<\/a> (<a href=\"http:\/\/pharmrev.aspetjournals.org\/content\/61\/2\/162.full\">R<\/a>). Low hypothalamic serotonin leads to increased carb cravings. Low orexin leads to decreased appetite.<\/p>\n<p>8) Decreased sex drive. The ventromedial (<a href=\"http:\/\/en.wikipedia.org\/wiki\/Ventromedial_nucleus#cite_note-15\">R<\/a>), arcuate and anterior nucleus are the portions of the hypothalamus that controls sex drive. Dopamine in the hypothalamus is largely responsible for sex drive (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19453889\">R<\/a>). CRH, produced by the hypothalamus, also inhibits<a href=\"https:\/\/selfhacked.com\/blog\/gonadotropin-releasing-hormone\/\"> GnRH<\/a>, which is largely responsible for sex drive.<\/p>\n<p>9) Fear\/Anxiety\/Stress\/<a href=\"https:\/\/selfhacked.com\/blog\/obsessive-compulsive-disorder-ocd\/\">OCD<\/a>\/Overly Emotional. The picture below shows how the hypothalamus executes fear from the amygdala.
