Read on to find out more about nebulizers, how they work, what are the most common uses, and how to use them safely.<\/p>\n<p><!--more--><\/p>\n<h2><span id=\"What_is_a_Nebulizer\"><a id=\"post-50736-_erl96mvukzll\"><\/a><strong>What is a Nebulizer?<\/strong><\/span><\/h2>\n<p>A nebulizer is a machine that turns water-based solutions into drops that are inhaled and small enough to reach the lungs (1 to 5 \u03bcm). It is usually used with drugs that help expand the lung passages, bronchi (bronchodilators). The direct delivery of the medication to the lungs reduces the dose and side effects [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10894454\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/25534396\"> R<\/a>].<\/p>\n<p>The droplets are generated using different methods: air-blast atomization, high-frequency vibrations, and colliding liquid jets (more information is in the Components section). These often determine the kind of drugs used with the device and its cost [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/25534396\">R<\/a>].<\/p>\n<h3><span id=\"Who_Should_Use_It\"><a id=\"post-50736-_gcyk66x17gnu\"><\/a>Who Should Use It?<\/span><\/h3>\n<p>Nebulizers are mostly used in patients that cannot use a specific breathing technique or coordinate their movements (hand-eye-breath coordination), such as in young children and the elderly. They are easily used as long as the person is breathing normally [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15294070\">R<\/a>].<\/p>\n<p>The use of home nebulizers is also recommended for patients with frequent airflow obstruction or low volume of breathing, or for those that need medications or doses not available for regular inhalers (dry-powder or metered-dose inhalers) [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/8617094\">R<\/a>].<\/p>\n<h3><span id=\"Mechanism_of_Action\"><a id=\"post-50736-_dmfhvisrkuv0\"><\/a>Mechanism of Action<\/span><\/h3>\n<p>Drops with a size 2 to 5 \u03bcm reach the higher parts of the airway while smaller drops (1 to 2 \u03bcm) can reach deeper [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23522986\">R<\/a>].<\/p>\n<p>They reach the airways in different ways, such as by:<\/p>\n<ul>\n<li><strong>Impaction<\/strong>, when big drops with a high-speed crash with the walls of the airway, usually in the upper parts. This also happens during exhalation [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23522986\">R<\/a>]<\/li>\n<li><strong>Sedimentation, <\/strong>which usually happens in the middle of the airway, where particles at a lower speed settle on the surface due to gravity. This is increased by holding your breath [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23522986\">R<\/a>]<\/li>\n<li><strong>Diffusion,<\/strong> the random movement of very small particles that eventually causes them to crash with the walls of the airway [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23522986\">R<\/a>]<\/li>\n<\/ul>\n<p>The nebulized drug must pass several barriers to be able to reach its target. In the upper airway, a layer of mucus covers the surface and removes foreign particles by \u201cpushing\u201d them out into the throat to be swallowed [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9155846\">R<\/a>].<\/p>\n<p>In the lower airway,<a href=\"https:\/\/selfhacked.com\/blog\/how-to-increase-and-decrease-white-blood-cells\/\"> white blood cells<\/a> (macrophages) \u201ceat\u201d foreign particles 1 to 2 \u03bcm in size. The nebulized drug must be small enough to reach the lower airway, and have a chemical composition that interferes with sticking to the mucus or that helps cross it [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21255854\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9155846\"> R<\/a>].<\/p>\n<p>The use of a mask is recommended for conditions such as allergic rhinitis. While both <strong>help reduces the loss of medicine, <\/strong>their different designs also help minimize treatment times or deliver the desired dose more precisely [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22472501\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15539736\"> R<\/a>].<\/p>\n<p>The main disadvantage of jet nebulizers is that they are noisy. Use of the nebulized antibiotic tobramycin twice a day can be used, or alternatively, azithromycin or aztreonam [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/28184305\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23540878\"> R<\/a>].<\/p>\n<h4><a id=\"post-50736-_ym87wu2kt4yn\"><\/a><strong>Immunocompromised Patients<\/strong><\/h4>\n<p>Pneumonia can be fatal for patients with a very weakened<a href=\"https:\/\/selfhacked.com\/blog\/primer-immune-system-works-videosimages\/\"> immune system<\/a>, such as HIV positive patients, those with leukemia, or those who have undergone organ or stem cell transplants [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/27550992\">R<\/a>].<\/p>\n<p>Nebulized pentamidine, a strong anti-infective drug, is used to prevent protozoa (<em>Pneumocystis<\/em> <em>jiroveci) <\/em>infections in people with compromised immunity (including transplant patients allergic to trimethoprim\/sulfamethoxazole) [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/28782390\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/27550992\"> R<\/a>].<\/p>\n<p>Pneumonia can also be caused by a virus, such as the respiratory syncytial virus. Nebulized ribavirin, an antiviral drug, is approved for use in patients with a compromised immune system [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26068871\">R<\/a>].<\/p>\n<p>The use of nebulized liposomal amphotericin B in patients who received a lung transplant to avoid infection (<em>Aspergillus <\/em>spp.) proved effective in a study of 412 patients (retrospective observational) [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26339864\">R<\/a>].<\/p>\n<h4><a id=\"post-50736-_vjd4chxfn75x\"><\/a><strong>Mechanically Ventilated Patients<\/strong><\/h4>\n<p>Nebulized antibiotics (like colistin or aminoglycosides), antivirals, or antifungals are advised <strong>only if conventional (IV) therapy is not an option<\/strong> for mechanically ventilated patients; although, in practice, it is used in hospitals around the world [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/28412382\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26723563\"> R<\/a>].<\/p>\n<h3><span id=\"4_Chronic_Obstructive_Pulmonary_Disease\"><a id=\"post-50736-_o602yd93wi72\"><\/a><strong>4) Chronic Obstructive Pulmonary Disease<\/strong><\/span><\/h3>\n<p><strong>Chronic Obstructive Pulmonary Disease<\/strong> (COPD) is a progressive condition that makes breathing difficult for the patient. Most patients have inflammation of the bronchial tubes, damage to the air sacs, and excessive production of mucus [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10911010\">R<\/a>].<\/p>\n<p><strong>Nebulized furosemide,<\/strong> a diuretic<strong>, <\/strong>helped relieve difficulty in breathing in 2 double-blind randomized controlled trials of 20 and 100 COPD patients [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/18250181\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23650431\"> R<\/a>].<\/p>\n<p>Nebulized fentanyl citrate or low doses of nebulized morphine helped improve breathing during<a href=\"https:\/\/selfhacked.com\/blog\/top-14-proven-health-benefits-exercise-references-mechanisms\/\"> exercise<\/a> in 23 patients (9 with advanced COPD) in 2 trials. However, nebulized morphine resulted in no improvement in breathing in different double-blind randomized controlled trials with COPD patients [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22168961\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/2669222\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9387952\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/8325064\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/7638804\"> R<\/a>].<\/p>\n<h3><span id=\"5_Acute_Lung_Injury\"><a id=\"post-50736-_3scj02n7qaoi\"><\/a><strong>5) Acute Lung Injury<\/strong><\/span><\/h3>\n<p>Often, bleeding disorders with increased coagulation occur after acute lung injury.
