Read on to learn about why and how grapefruit messes with your medications, and what are the risks and side effects of these interactions.<\/p>\n<p><!--more--><\/p>\n<h2><span id=\"Scientific_Research_on_Grapefruit\"><a id=\"post-44563-_9apss7szm01i\"><\/a><strong>Scientific Research on Grapefruit<\/strong><\/span><\/h2>\n<p>In the early 1990s, scientists found that<a href=\"https:\/\/selfhacked.com\/blog\/grapefruit\/\"> <strong>grapefruit<\/strong><\/a><strong> juice could markedly increase the levels and action of drugs taken orally. <\/strong>This discovery led to further investigations of grapefruit\u2019s constituents, drug interactions, and the mechanisms involved [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/2612087\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2147024\/\">R<\/a>].<\/p>\n<p>Drugs that interact with one or more of grapefruit\u2019s many biologically active substances are increasingly available. Many are frequently prescribed and essential for treating common medical conditions [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>].<\/p>\n<p>Of concern is that new drugs tend to be released before grapefruit interaction testing, putting patients at risk [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23044937\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21648217\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3612156\/\"> R<\/a>].<\/p>\n<p>However, some scientists believe that only a few drugs can produce significant reactions (these include simvastatin,<a href=\"https:\/\/selfhacked.com\/blog\/6-buspirone-uses-side-effects-7-natural-alternatives\/\"> buspirone<\/a>, felodipine, and possibly cyclosporine). This is because many of the interaction studies involve exposure to quantities of grapefruit juice far higher than that normally consumed [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3071161\/\">R<\/a>].<\/p>\n<p>They claim that the risk of grapefruit interactions with many other drugs, including quinine, quinidine, erythromycin, dextromethorphan, oxycodone, tolvaptan, nilotinib, and sunitinib, are clinically not relevant [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3612155\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21142260\"> R<\/a>].<\/p>\n<h2><span id=\"Mechanisms_of_the_Grapefruit_Effect\"><a id=\"post-44563-_pcykd8u155zo\"><\/a><strong>Mechanisms of the &#8220;Grapefruit Effect&#8221;<\/strong><\/span><\/h2>\n<h3><span id=\"Why_and_How_Grapefruit_Messes_with_Your_Medications\"><a id=\"post-44563-_nr4y4sy1andx\"><\/a><strong>Why and How Grapefruit Messes with Your Medications<\/strong><\/span><\/h3>\n<p>There are 3 main ways in which grapefruit interacts with drugs; however, these can also be modified by genetics, and vary from person to person [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23297175\">R<\/a>].<\/p>\n<h4><a id=\"post-44563-_hisbmd7tklih\"><\/a><strong>1) CYP3A4 Inhibition<\/strong><\/h4>\n<p><strong><img loading=\"lazy\" width=\"704\" height=\"464\" class=\"wp-image-90422\" src=\"https:\/\/selfhacked.com\/blog\/app\/uploads\/2018\/01\/word-image.jpeg\" srcset=\"https:\/\/selfhacked.com\/app\/uploads\/2018\/01\/word-image.jpeg 704w, https:\/\/selfhacked.com\/app\/uploads\/2018\/01\/word-image-480x316.jpeg 480w\" sizes=\"(max-width: 704px) 100vw, 704px\" \/><\/strong><\/p>\n<p>Furanocoumarins found in grapefruit block the<a href=\"https:\/\/selfdecode.com\/gene\/cyp3a4\/\"> CYP3A4<\/a> enzyme. This is an enzyme found in the small intestine and the liver that breaks down (metabolizes) drugs. It is responsible for processing about 50% of all prescribed drugs [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5287983\/\">R<\/a>].<\/p>\n<p><strong>When grapefruit juice is consumed,<\/strong><a href=\"https:\/\/selfdecode.com\/gene\/cyp3a4\/\"><strong> CYP3A4<\/strong><\/a><strong>&#8216;s ability to break down the drug is decreased<\/strong> [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17112309\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9723817\"> R<\/a>].<\/p>\n<p>This can result in:<\/p>\n<ul>\n<li><strong>Stronger effects<\/strong>, due to higher blood levels of the drug. Also, more side effects.<\/li>\n<li><strong>Weaker effects<\/strong>. This would happen when the <strong>drug you take actually needs<\/strong><a href=\"https:\/\/selfdecode.com\/gene\/cyp3a4\/\"><strong> CYP3A4<\/strong><\/a><strong> to transform it into the active compound<\/strong> (e.g. amiodarone, clopidogrel).<\/li>\n<\/ul>\n<p>Apart from<a href=\"https:\/\/selfdecode.com\/gene\/cyp3a4\/\"> CYP3A4<\/a>, grapefruit components can also block<a href=\"https:\/\/selfhacked.com\/blog\/cyp1a2\/\"> CYP1A2<\/a>,<a href=\"https:\/\/selfhacked.com\/blog\/cyp2a6\/\"> CYP2A6<\/a>,<a href=\"https:\/\/selfhacked.com\/blog\/cyp2c9\/\"> CYP2C9<\/a>,<a href=\"https:\/\/selfhacked.com\/blog\/cyp2c19\/\"> CYP2C19<\/a>, and<a href=\"https:\/\/selfhacked.com\/blog\/cyp2d6\/\"> CYP2D6<\/a> [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17112808\">R<\/a>].<\/p>\n<p>Scientists think that grapefruit also likely blocks<a href=\"https:\/\/selfhacked.com\/blog\/cyp3a5\/\"> CYP3A5<\/a> with the same intensity as<a href=\"https:\/\/selfdecode.com\/gene\/cyp3a4\/\"> CYP3A4<\/a> (these enzymes have highly similar functions), but this hasn&#8217;t been properly studied [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20160155\">R<\/a>].<\/p>\n<h4><a id=\"post-44563-_ex26funvszhg\"><\/a><strong>2) P-glycoprotein Pump Inhibition\/Activation<\/strong><\/h4>\n<p>P-glycoprotein pump or Pgp (<a href=\"https:\/\/selfdecode.com\/gene\/mdr1\/\">MDR1<\/a>,<a href=\"https:\/\/selfdecode.com\/gene\/abcb1\"> ABCB1<\/a>) is found in the gut lining, where it pumps drugs back into the gut. It is also found in the liver, kidneys, blood-brain and blood-testis barrier. It has an important function in protecting our body from foreign chemicals, such as drugs and toxins.<\/p>\n<p>Several studies suggest that grapefruit blocks the pump by both decreasing its production and blocking its function. This would increase the amount of the drug that makes it into the bloodstream and enhance the effect drugs can have on the body [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15504753\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15531381\"> R<\/a>].<\/p>\n<p>However, other studies suggest that grapefruit juice may also activate the pump, which would mean a greater flow of the drug back into the gut and away from the blood [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2147024\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10227700\"> R<\/a>].<\/p>\n<p>In addition, the interaction is more complex, because these effects can be offset\/augmented by grapefruit also blocking the CYP3A enzymes [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10227700\">R<\/a>].<\/p>\n<p>So, it is<strong> not completely understood how grapefruit juice affects transport across the pump<\/strong>. In addition, a scientific review on the topic <strong>suggests that grapefruit might have a &#8220;minimal&#8221; effect on drug absorption in the body<\/strong> [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3071161\/\">R<\/a>].<\/p>\n<h4><a id=\"post-44563-_d2vfg0p5185c\"><\/a><strong>3) OATP Inhibition<\/strong><\/h4>\n<p>OATPs (organic anion transporting polypeptides) are cell transporters involved in the transport of a number of widely used drugs.<\/p>\n<p>Grapefruit may block OATPs (including<a href=\"https:\/\/selfdecode.com\/gene\/slco1a2\/\"> OATP1A2<\/a>,<a href=\"https:\/\/selfdecode.com\/gene\/slco1b1\/\"> OATP1B1<\/a>, and<a href=\"https:\/\/selfdecode.com\/gene\/slco2b1\/\"> OATP2B1<\/a>), and thereby decrease the transport of the drug from the gut into the bloodstream. This decreases drug availability [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11823753\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3071161\/\"> R<\/a>].<\/p>\n<h3><span id=\"Grapefruit_Compounds_Responsible_for_these_Effects\"><a id=\"post-44563-_wjz0u1afko3p\"><\/a><strong>Grapefruit Compounds Responsible for these Effects<\/strong><\/span><\/h3>\n<p>Grapefruit comes in many varieties with different levels of active components depending on the variety, maturity, origin of the fruit, local climate conditions, and the manufacturing process. No single component accounts for all observed interactions [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23373097\">R<\/a>].<\/p>\n<p>The compounds thought to be responsible for interfering with drugs include [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2147024\/\">R<\/a>]:<\/p>\n<ul>\n<li>Flavonoid glycosides (narirutin, naringin, naringenin,<a href=\"https:\/\/selfhacked.com\/blog\/22-scientifically-proven-benefits-quercetin\/\"> quercetin<\/a>, kaempferol,<a href=\"https:\/\/selfhacked.com\/blog\/hesperidin\/\"> hesperidin<\/a>, neohesperidin, didymin, and poncirin)<\/li>\n<li>Furanocoumarins (6&#8242;,7&#8242;-dihydroxybergamottin, bergamottin)<\/li>\n<li>Sesquiterpenes (nootkatone)<\/li>\n<\/ul>\n<p>Several studies have shown DHB (6&#8242;,7&#8242;-dihydroxybergamottin) and to an extent, bergamottin to be important contributors to the grapefruit juice effect (blocking CYP3A enzymes) [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15179411\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10859150\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12891222\"> R<\/a>].<\/p>\n<p>On the other hand, the flavonoids, especially naringin, are the grapefruit constituents responsible for clinically significant OATP inhibition. In contrast to CYP3A, OATP inhibition is short-lived, and the reaction can be avoided by taking the drug and grapefruit at different times [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3071161\/\">R<\/a>].<\/p>\n<p>The complex chemistry of grapefruit juice makes its use in drug interaction studies challenging. The concentrations of bergamottin, DHB, and naringin that produce adverse effects are often not reported [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3071161\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17301733\"> R<\/a>].<\/p>\n<p>There is some effort to produce grapefruit juice without furanocoumarins by removing these via methods such as filtration, heating, or<a href=\"https:\/\/selfhacked.com\/blog\/avoiding-sun-will-kill-14-proven-science-based-health-benefits-sun\/\"> UV<\/a> exposure. This would potentially create \u201cinteraction-free\u201d grapefruit juice that doesn\u2019t interact with drugs. The question, however, is whether this juice would provide the same <a href=\"https:\/\/selfhacked.com\/blog\/grapefruit\/\">health benefits<\/a> as the unprocessed one [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3071161\/\">R<\/a>].<\/p>\n<h3><span id=\"What_Determines_Which_Drugs_Are_Affected\"><a id=\"post-44563-_2jrhdemfxz8m\"><\/a><strong>What Determines Which Drugs Are Affected?<\/strong><\/span><\/h3>\n<p>The interaction between medications and grapefruit is drug-specific. Each drug is broken down and passes through the body in different ways.<\/p>\n<p>Affected drugs possess 3 essential characteristics:<\/p>\n<ul>\n<li><strong>They are taken by mouth.<\/strong> Enzyme\/drug interaction occurs mostly in the intestines, rather than the liver. The same drug may be well tolerated if given intravenously [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2147024\/\">R<\/a>].<\/li>\n<li>Only a low fraction of the drug reaches the blood [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>]<\/li>\n<li>They are metabolized by<a href=\"https:\/\/selfdecode.com\/gene\/cyp3a4\/\"> CYP3A4<\/a> (or transported by OATP or Pgp) [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>]<\/li>\n<\/ul>\n<h3><span id=\"What_Determines_the_Magnitude_of_the_Interaction\"><a id=\"post-44563-_copy9o25sq07\"><\/a><strong>What Determines the Magnitude of the Interaction?<\/strong><\/span><\/h3>\n<ul>\n<li>The seriousness of the dose-related drug toxicity (and side effects) [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>]<\/li>\n<li>The extent to which the drug concentration increases in the blood [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>]<\/li>\n<li>The timing and type of grapefruit product taken (whole, juice, seed extract) [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>]<\/li>\n<li>Age, ethnicity, and genetics [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>]<\/li>\n<\/ul>\n<p>Generally speaking, a grapefruit-drug interaction is likely to be of clinical importance <strong>if the drug has a narrow therapeutic range, or when the drug&#8217;s movement through the body is dramatically altered by grapefruit<\/strong> [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3071161\/\">R<\/a>].<\/p>\n<h2><span id=\"Why_Is_Grapefruit-Drug_Interaction_still_a_Problem\"><a id=\"post-44563-_60anrs4rsbwx\"><\/a><strong>Why Is Grapefruit-Drug Interaction still a Problem?<\/strong><\/span><\/h2>\n<p>Unless health professionals are aware of the recent addition of grapefruit to a patient&#8217;s diet, they may not investigate grapefruit as a cause of a negative drug reaction. Also, patients may not volunteer this information and many of them don\u2019t realize it is of importance [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>].<\/p>\n<p>That is why some scientists think that current data does not provide an absolute or even approximate number of the true incidence of grapefruit-drug interactions [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\">R<\/a>].<\/p>\n<h2><span id=\"Who_is_at_Higher_Risk_of_Grapefruit-Drug_Interactions\"><a id=\"post-44563-_3zfklv60n92t\"><\/a><strong>Who is at Higher Risk of Grapefruit-Drug Interactions?<\/strong><\/span><\/h2>\n<p>Older people are especially at risk for grapefruit-drug interactions because [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17125569\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/18382075\"> R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10945313\">R<\/a>]:<\/p>\n<ul>\n<li>People older than 45 years are the prime purchasers of grapefruit<\/li>\n<li>They also receive the most drug prescriptions<\/li>\n<li>Older adults are less able to tolerate excessive drug levels in the blood<\/li>\n<\/ul>\n<p>Also at risk are people with certain genetic differences that impact the production of the enzymes<a href=\"https:\/\/selfdecode.com\/gene\/cyp3a4\/\"> CYP3A4<\/a>\/<a href=\"https:\/\/selfdecode.com\/gene\/cyp3a5\">CYP3A5<\/a> [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2147024\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1873672\/\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20160155\"> R<\/a>].<\/p>\n<h2><span id=\"Drugs_that_Interact_with_Grapefruit\"><a id=\"post-44563-_2r77bjoiadlw\"><\/a><strong>Drugs that Interact with Grapefruit<\/strong><\/span><\/h2>\n<p>Below is a list of drugs that interact with grapefruit, based on data gathered from 2012 through 2013. Many new drugs not listed will also produce a similar risk. Also, drugs have different trade names in different countries [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/table\/t1-1850309\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3589309\/\"> R<\/a>].<\/p>\n<h3><span id=\"Grapefruit_DECREASES_the_Effects_of_These_Drugs\"><a id=\"post-44563-_u2ck75aefeha\"><\/a><strong>Grapefruit DECREASES the Effects of These Drugs<\/strong><\/span><\/h3>\n<h4><a id=\"post-44563-_neuwtpt8e3u0\"><\/a><strong>Antiarrhythmic drugs<\/strong><\/h4>\n<ul>\n<li><strong>Amiodarone (Cordarone, Nextrone)<\/strong>. When amiodarone is broken down by CYP3A the product, N-desethylamiodarone (N-DEA), is stronger than the original drug. Grapefruit juice almost completely blocks this conversion [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16048354\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10759694\"> R<\/a>].<\/li>\n<\/ul>\n<h4><a id=\"post-44563-_l72mjv78e3we\"><\/a><strong>Antibiotics<\/strong><\/h4>\n<ul>\n<li>Ciprofloxacin (Cipro, Cipro XR). Orange juice decreases the levels of this drug, and grapefruit juice may have a similar effect [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11936572\">R<\/a>].<\/li>\n<\/ul>\n<h4><a id=\"post-44563-_agh0u0s115b9\"><\/a><strong>Antihistamines<\/strong><\/h4>\n<ul>\n<li>Fexofenadine (Allegra). This non-drowsy antihistamine can interact not only with grapefruit juice but also with apples and orange juice. Blood levels of the drug go down (due to OATP inhibition) and the effectiveness may be reduced. A study of healthy adults shows that the absorption of fexofenadine is reduced by 30% when taken with grapefruit juice [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9723817\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11978146\"> R<\/a>].<\/li>\n<\/ul>\n<h4><a id=\"post-44563-_404ijbc7j1wc\"><\/a><strong>Antiplatelet Drugs (Against Blood Clots)<\/strong><\/h4>\n<ul>\n<li>Clopidogrel (Plavix). Grapefruit juice decreases drug levels and can markedly decrease the platelet-inhibitory effect of clopidogrel in human volunteers [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24067745\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/25848291\"> R<\/a>].<\/li>\n<li>Prasugrel (Effient) [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/25557052\">R<\/a>].<\/li>\n<\/ul>\n<h4><a id=\"post-44563-_tbqczs9sun4j\"><\/a><strong>Beta Blockers<\/strong><\/h4>\n<ul>\n<li>Acebutolol (Sectral, Prent). Due to<a href=\"https:\/\/selfdecode.com\/gene\/slco1a2\/\"> OATP1A2<\/a> inhibition [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19067419\">R<\/a>].<\/li>\n<li>Atenolol (Tenormin). Due to OATP1A2 inhibition [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19067419\">R<\/a>].<\/li>\n<li>Sotalol (Betapace). Due to OATP1A2 inhibition [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19067419\">R<\/a>].<\/li>\n<\/ul>\n<h4><a id=\"post-44563-_yfcw94x0ocq9\"><\/a><strong>Birth Control Pills<\/strong><\/h4>\n<ul>\n<li><strong>Oral contraceptives<\/strong>. Grapefruit may decrease the breakdown of estrogens. In a randomized crossover study of 13 healthy volunteers, grapefruit juice increased the fraction of<a href=\"https:\/\/selfhacked.com\/blog\/estradiol\/\"> estrogen<\/a> that reached the blood.
