These are <strong>enzymes that eliminate most of the drugs and toxins<\/strong> from the human body (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4164225\/\">R<\/a>).<\/p>\n<p><a href=\"https:\/\/selfhacked.com\/blog\/cyp-enzymes-interact-supplements-related-genes\/\">Read more about CYPs here.<\/a><\/p>\n<p>CYP2C9 is responsible for the metabolic clearance of up to 15-20% of all drugs undergoing Phase I <a href=\"https:\/\/selfhacked.com\/blog\/the-science-of-detoxification\/\">detoxification<\/a> (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>).<\/p>\n<p>This enzyme is the third most important cytochrome P450 (CYP) in terms of the number of drugs metabolized (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16863464\">R<\/a>).<\/p>\n<h3><span id=\"CYP2C9_Function\"><strong>CYP2C9 Function<\/strong><\/span><\/h3>\n<p>This enzyme metabolizes:<\/p>\n<ul>\n<li>Antidiabetics: tolbutamide (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26886310\">R<\/a>), nateglinide (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>) and glipizide (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>)<\/li>\n<li>Antiepileptics: phenytoin (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>) and valproate (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4588450\/\">R<\/a>). CYP2C9 is responsible for about 90% of phenytoin metabolism (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>)<\/li>\n<li>Antihypertensive losartan (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23118328\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4246459\/\">R<\/a>)<\/li>\n<li>Cannabinol (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17303175\">R<\/a>)<\/li>\n<li>Non-steroidal anti-inflammatory drugs (NSAIDs): flurbiprofen (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/25712887\">R<\/a>), ibuprofen (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26122864\">R<\/a>), and celecoxib (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/27864660\">R<\/a>)<\/li>\n<li>Anticoagulants: warfarin, acenocoumarol, phenprocoumon (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16863464\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4134937\/\">R<\/a>)<\/li>\n<li>Anti-asthmatic zafirlukast (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/27377818\">R<\/a>)<\/li>\n<li><a href=\"https:\/\/selfhacked.com\/blog\/cholesterol\/\">Cholesterol<\/a>-lowering fluvastatin (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>)<\/li>\n<\/ul>\n<p>CYP2C9 also metabolizes many hormones and other compounds, such as <a href=\"http:\/\/selfhacked.com\/blog\/top-21-science-based-health-benefits-progesterone-4-drawbacks\/\">progesterone<\/a>, <a href=\"https:\/\/selfhacked.com\/blog\/testosterone-will-add\/\">testosterone<\/a> and <a href=\"https:\/\/selfhacked.com\/blog\/arachidonic-acid\/\">arachidonic acid<\/a> (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4843492\/\">R<\/a>).<\/p>\n<p>This enzyme is primarily found in the liver (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>), heart and blood vessels (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4246459\/\">R<\/a>).<\/p>\n<p>CYP2C9 accounts for about 20% of the total liver CYP content (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4843492\/\">R<\/a>).<\/p>\n<h3><span id=\"Beneficial_Effects\">Beneficial Effects<\/span><\/h3>\n<p>In the human heart, CYP2C9 produces epoxyeicosatrienoic acids (EETs) from arachidonic acid (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4246459\/\">R<\/a>).<\/p>\n<p>EETs have a protective effect on blood vessels, including vasodilatation (widening of the blood vessels), anti-inflammatory, and antithrombotic effects. They are also believed to protect the heart by increasing post-ischemic (<a href=\"http:\/\/selfhacked.com\/blog\/why-you-may-be-suffering-from-low-oxygen-and-not-realize-it\/\">low oxygen<\/a>) function and reducing heart infarct size (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4246459\/\">R<\/a>).<\/p>\n<p>People with higher CYP2C9 activity were less likely to develop colorectal adenoma (colon cancer) in a study of 928 subjects (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3529799\/\">R<\/a>). This suggests that the activity of this enzyme is protective against cancer.<\/p>\n<h3><span id=\"Detrimental_Effects\">Detrimental Effects<\/span><\/h3>\n<p>This enzyme can activate some cancer-causing agents found in cigarette smoke. Smokers with lower enzyme activity were less likely to develop colorectal adenoma (colon cancer) than smokers with higher enzyme activity, according to a study of 928 subjects (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3529799\/\">R<\/a>).<\/p>\n<h2><span id=\"Genetic_Variations\"><strong>Genetic Variations<\/strong><\/span><\/h2>\n<p>More than 67 variants of CYP2C9 have been identified (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4843492\/\">R<\/a>). Many of these decrease enzyme activity.<\/p>\n<p>Patients with low enzyme activity are at greater risk of developing adverse reactions, especially for CYP2C9-processed drugs with a narrow therapeutic window, such as S-warfarin, phenytoin, glipizide, and tolbutamide (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26037375\">R<\/a>).<\/p>\n<h3><span id=\"rs1799853\">rs1799853<\/span><\/h3>\n<p>The <a href=\"https:\/\/selfdecode.com\/snp\/rs1799853\/\">rs1799853<\/a> T variant at this position reduces CYP2C9 activity by approximately 50% (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>). It is also known as the CYP2C9*2 variant.<\/p>\n<p>The T variant is found in about 10-20% of Whites, but is rare in Asian and African-American populations (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>).<\/p>\n<p>Patients with the CYP2C9*2 variant require lower doses of warfarin to achieve a similar anticoagulant effect as other patients (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>), and also lower doses of other medication metabolized by this enzyme.<\/p>\n<p>Individuals with this variant are at risk of prolonged bleeding and more frequent severe bleeding with warfarin therapy. They have a higher possibility of low blood sugar levels during glipizide and tolbutamide therapy. Finally, they have more frequent symptoms of overdose in phenytoin therapy (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4843492\/\">R<\/a>).<\/p>\n<p>People with this variant also show an increased risk of developing acute gut bleeding during the use of NSAIDs (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19422321\">R<\/a>).<\/p>\n<p>People with this variant have reduced bone mineral density, which could contribute to developing osteoporosis (pilot study, 92 subjects) (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23732294\">R<\/a>).<\/p>\n<p>This variant was also associated with colorectal adenoma (colon cancer) in a study of 928 subjects (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3529799\/\">R<\/a>).<\/p>\n<h3><span id=\"rs1057910\">rs1057910<\/span><\/h3>\n<p><a href=\"https:\/\/selfdecode.com\/snp\/rs1057910\/\">rs1057910<\/a> C at this position significantly reduces the activity of the enzyme (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>).<\/p>\n<p>This variant has much lower frequencies in African and Asian populations compared with Caucasians (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>). Relatively high frequency has been found among South Asians (11.7%) (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26595139\">R<\/a>).<\/p>\n<p>Patients require lower doses of warfarin, celecoxib, and other CYP2C9-metabolized drugs (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23996211\">R<\/a>).<\/p>\n<p>These variants are associated with prolonged bleeding and more frequent severe bleeding on warfarin therapy. They have a higher possibility of low blood sugar levels during glipizide and tolbutamide therapy. They also have more frequent symptoms of overdose in phenytoin therapy (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3201766\/\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/25994870\">R<\/a>).<\/p>\n<p>People with this variant were also more likely to develop acute gut bleeding during the use of NSAIDs (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19422321\">R<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4704657\/\">R<\/a>).<\/p>\n<p>This variant may be associated with vitiligo susceptibility, according to a study of 181 subjects (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4069645\/\">R<\/a>).<\/p>\n<p>People with this variant had reduced bone mineral density, which could contribute to developing osteoporosis, in a pilot study of 92 subjects (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23732294\">R<\/a>).<\/p>\n<p>Men carrying the C variant were more likely to have a heart attack (myocardial infarction).
