It has the same effect as commonly-used NSAIDs with fewer side effects [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/3318441\/\">R<\/a>, <a href=\"https:\/\/www.mayoclinic.org\/drugs-supplements-same\/art-20364924\">R<\/a>].<\/p>\n<p>In a Cochrane review of four trials with 656 patients, SAM-e <b>moderately improved pain and joint function<\/b>, compared with placebo. However, the authors pointed to certain flaws in study design discussed in \u201c<b>Limitations and Caveats<\/b>\u201d [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19821403\">R<\/a>].<\/p>\n<p>Given that long-term treatment with NSAIDs and other painkillers may cause stomach ulcers and other adverse effects, SAM-e could be a safer yet equally effective alternative [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5772852\/\">R<\/a>].<\/p>\n<h3><span id=\"Possibly_Effective\">Possibly Effective:<\/span><\/h3>\n<h3><span id=\"2_Depression\"><b>2) Depression<\/b><\/span><\/h3>\n<p>Depression rates keep growing, and standard treatment options sometimes fail to provide relief. Some people with depression have high homocysteine levels due to poor methylation [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3181767\/\">R<\/a>].<\/p>\n<p>Vitamin B12 and folate deficiencies can impair SAM-e production and, in turn, raise homocysteine and lower serotonin and noradrenaline [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1737050\/\">R<\/a>].<\/p>\n<p>SAM-e is one of the most studied complementary treatments for depression; extensive clinical reviews have documented its safety and efficacy. It acts faster than common antidepressants and yields similar results [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12420702\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19909689\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26295824\"> R<\/a>].<\/p>\n<p>A large evidence-based database, Cochrane, reviewed eight clinical trials with over 900 depressed patients. They concluded that SAM-e works as well as antidepressant drugs and noted its excellent safety profile. However, <b>neither of the treatment options was significantly better than placebo<\/b> [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6457972\/\">R<\/a>].<\/p>\n<p>In a recent study of 60 patients with mild-to-moderate depression, a combination of SAM-e and vitamin B complex improved the symptoms while causing no side effects [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6247995\/\">R<\/a>].<\/p>\n<p>According to four studies with over 170 patients, SAM-e (800-1,600 mg daily) can even combat drug-resistant depression when added to standard treatment. It provided significant improvement in <b>35-60%<\/b> of the patients [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20595412\">R<\/a>,<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2212958815000415\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3666233\/\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15538131\/\"> R<\/a>].<\/p>\n<p>Please note that the health authorities advise against combining SAM-e with antidepressants, due to potential drug interactions (see \u201c<b>Drug Interactions<\/b>\u201d below for more details) [<a href=\"https:\/\/www.mayoclinic.org\/drugs-supplements-same\/art-20364924\">R<\/a>].<\/p>\n<p>The antidepressant effects of SAM-e may be more pronounced in men [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5380442\/\">R<\/a>].<\/p>\n<p>Besides improving the main symptoms of depression, SAM-e also enhanced cognition in these patients and reversed sexual dysfunction (caused by antidepressants) in some of these trials [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21665441\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21398094\/\"> R<\/a>].<\/p>\n<div class=\"sh-summary\"><div class=\"sh-summary-content\">SAM-e may help with mild-to-moderate depression, but its effectiveness is comparable to placebo. It acts faster and has fewer side effects than conventional antidepressants.<\/div><\/div>\n<h3><span id=\"3_Liver_Protection\"><b>3) Liver Protection<\/b><\/span><\/h3>\n<p>As mentioned, S-adenosylmethionine boosts the master antioxidant, glutathione. Glutathione guards the liver against different chemicals, toxic metabolites, and oxidative stress [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12163146\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12163147\"> R<\/a>].<\/p>\n<p>In a meta-analysis of 12 clinical trials (705 patients), SAM-e significantly reduced two markers of liver damage:<a href=\"https:\/\/selfhacked.com\/blog\/bilirubin-benefit-how-to-optimize\/\"> bilirubin<\/a> and<a href=\"https:\/\/selfhacked.com\/blog\/aspartate-aminotransferase-ast\/\"> AST<\/a>. The researchers observed these effects in patients with [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4361566\/\">R<\/a>]:<\/p>\n<ul>\n<li>Liver scarring (cirrhosis)<\/li>\n<li>Impaired bile flow (cholestasis)<\/li>\n<li>Alcoholic liver disease<\/li>\n<li>Liver cancer<\/li>\n<\/ul>\n<p>That doesn\u2019t necessarily mean that SAM-e can treat the above conditions.<\/p>\n<p>Overall, SAM-e was safe and provided moderate benefits, but most patients responded even better to ursodeoxycholic acid (ursodiol).<\/p>\n<p>SAM-e may also reverse<a href=\"https:\/\/selfhacked.com\/blog\/cholesterol\/\"> cholesterol<\/a> buildup in the liver and shield it against toxic drugs such as acetaminophen (Tylenol) and steroids [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/1327655\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22659519\"> R<\/a>].<\/p>\n<h4><b>Bile Flow<\/b><\/h4>\n<p>Liver disease can lead to <b>cholestasis<\/b> or reduced<a href=\"https:\/\/selfhacked.com\/blog\/bile-supplements\/\"> bile<\/a> flow. Bile acids and toxic metabolites build up and can further damage liver cells. This condition can also occur during pregnancy and may raise the risk of birth complications [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4125628\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24901263\"> R<\/a>].<\/p>\n<p>According to two clinical reviews, SAM-e (1,600 mg daily) can improve markers of liver health in cholestasis and relieve symptoms such as skin itching, fatigue, and discomfort [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/8439634\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/2179653\"> R<\/a>].<\/p>\n<p>Either alone or in combination with ursodeoxycholic acid, it improved the symptoms of cholestasis in another three clinical trials with over 150 pregnant women [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5501081\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16965225\"> R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/2083923\/\"> R<\/a>].<\/p>\n<p>The same combination reduced birth complications from cholestasis in an analysis of 10 clinical trials (727 pregnant women). It lowered the rates of [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/25173231\/\">R<\/a>]:<\/p>\n<ul>\n<li>Cesarean section, by <b>55%<\/b><\/li>\n<li>Preterm birth, by <b>64%<\/b><\/li>\n<li>Fetal suffocation, by <b>73%<\/b><\/li>\n<\/ul>\n<p>The authors mentioned a few flaws in methodology as potential sources of bias in those trials.<\/p>\n<p>A smaller study with 18 pregnant women found no significant benefits [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/2050326\">R<\/a>].<\/p>\n<h4><b>Alcoholic Liver Disease<\/b><\/h4>\n<p>Chronic alcohol consumption massively increases oxidative stress and wreaks havoc in the liver. Natural supplements such as SAM-e and<a href=\"http:\/\/selfhacked.com\/blog\/milk-thistle\/\"> silymarin<\/a> have shown promising results in people with alcoholic liver disease [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4405238\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24212353\/\"> R<\/a>].<\/p>\n<p>In 123 people, SAM-e (1,200 mg daily for 2 years) cut the risk of death and liver transplantation. It offered greater benefits in earlier stages of alcoholic liver disease [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10406187\/\">R<\/a>].<\/p>\n<div class=\"sh-summary\"><div class=\"sh-summary-content\">SAM-e can boost antioxidant protection in the liver, stimulate bile flow, and reduce damage from alcohol and other toxins. Well-designed clinical trials should confirm these effects.<\/div><\/div>\n<h3><span id=\"4_Fibromyalgia\"><b>4) Fibromyalgia<\/b><\/span><\/h3>\n<p>Studies exploring complementary approaches to fibromyalgia are underway, and SAM-e is among them. According to a review of 70 clinical trials, SAM-e,<a href=\"http:\/\/selfhacked.com\/blog\/21-proven-science-based-reasons-magnesium-amazing-including-drawbacks\/\"> magnesium<\/a>, and <a href=\"https:\/\/selfhacked.com\/blog\/carnitine-top-18-science-based-health-effects-carnitine\/\">L-carnitine<\/a> showed the best results and the most potential for further research [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3503476\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20192908\"> R<\/a>].<\/p>\n<p>In 44 patients with fibromyalgia, SAM-e (800 mg daily for 6 weeks) reduced [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/1925418\">R<\/a>]:<\/p>\n<ul>\n<li>Muscle pain<\/li>\n<li>Fatigue<\/li>\n<li>Morning stiffness<\/li>\n<\/ul>\n<p>It also relieved depression and increased pain tolerance in 17 fibromyalgia patients. In another trial of 34 patients, injected SAM-e had no influence on pain; it did, however, slightly improve fatigue, sleep quality, and stiffness [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/3318438\/\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9225876\/\"> R<\/a>].<\/p>\n<h3><span id=\"Insufficient_Evidence\">Insufficient Evidence:<\/span><\/h3>\n<p><i>No valid clinical evidence supports the use of SAM-e for any of the conditions in this section. Below is a summary of up-to-date animal studies, cell-based research, or low-quality clinical trials which should spark further investigation. However, you shouldn\u2019t interpret them as supportive of any health benefit.<\/i><\/p>\n<h3><span id=\"5_Cognition_Enhancement\"><b>5) Cognition Enhancement<\/b><\/span><\/h3>\n<p>The beneficial effects of SAM-e on glutathione production may also shield the brain against oxidative damage caused by toxins or degenerative diseases [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11796196\">R<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10651146\"> R<\/a>].<\/p>\n<p>It reversed memory-related cognitive problems in 46 patients with drug-resistant depression [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21665441\">R<\/a>].<\/p>\n<p>In 12 healthy people, SAM-e supplementation (1,600 mg daily) supplied the brain with energy by boosting<a href=\"http:\/\/selfhacked.com\/blog\/creatine\/\"> creatine<\/a> production. This effect may underlie its ability to improve mood and cognition, but the lack of a placebo control prevents us from drawing conclusions [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/14550683\">R<\/a>].<\/p>\n<p>In mice deficient in<a href=\"https:\/\/selfhacked.com\/blog\/16-proven-benefits-vitamin-b9-folate-folic-acid\/\"> folate<\/a>, SAM-e restored<a href=\"https:\/\/selfhacked.com\/blog\/acetylcholine\/\"> acetylcholine<\/a> levels and enhanced cognitive performance. Interestingly enough, SAM-e also reduced the animals\u2019 aggressiveness. It worked by increasing<a href=\"http:\/\/selfhacked.com\/blog\/top-18-scientific-health-benefits-choline-cdp-choline-alpha-gpc\/\"> choline<\/a> availability in the brain [<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/18373034\">R<\/a>].<\/p>\n<p>The risk of folate deficiency increases with aging and impairs cognition.
