An incretin is a metabolic hormone that reduces <a href=\"https:\/\/selfhacked.com\/blog\/glucose\/\">glucose<\/a> levels in the blood after meals.<\/p>\n<h3><span id=\"Limitations\"><a id=\"post-19089-_kdsknmnd9gzp\"><\/a>Limitations<\/span><\/h3>\n<p>Note that GLP-1 is not the only hormone that affects blood sugar regulation and insulin production.<\/p>\n<p>Many other important enzymes like DPP-4, hormones &#8211; such as glucagon, growth hormone, epinephrine, cortisol, sex hormones, and thyroid hormones &#8211; and other pathways also impact blood sugar and insulin balance.<\/p>\n<p>Plus, blood sugar control can involve many other possible factors &#8211; including blood chemistry, environment, health status, and genetics &#8211; that may vary from one person to another.<\/p>\n<h3><span id=\"GLP-1_Agonists\"><a id=\"post-19089-_i35y6bmz08qh\"><\/a>GLP-1 Agonists<\/span><\/h3>\n<p>GLP-1 has received widespread scientific attention due to its involvement in insulin and blood sugar control. That\u2019s how a new class of medications was born, which are called GLP-1 agonists. These medications are typically given as injections (with the exception of semaglutide) [<a href=\"https:\/\/diabetes.diabetesjournals.org\/content\/51\/suppl_3\/S434\">R<\/a>].<\/p>\n<p>Doctors may prescribe GLP-1 agonists to people with type 2 diabetes who have poor blood sugar control and are taking anti-diabetic therapy by mouth.<\/p>\n<p>Approved GLP-1 agonists include:<\/p>\n<ul>\n<li>Byetta\/Bydureon (exenatide)<\/li>\n<li>Victoza (liraglutide)<\/li>\n<li>Lyxumia\/Adlyxin (lixisenatide)<\/li>\n<li>Tanzeum (albiglutide)<\/li>\n<li>Trulicity (dulaglutide)<\/li>\n<li>Ozempic (semaglutide injections)<\/li>\n<li>Rybelsus (oral semaglutide &#8211; the only oral GLP-1 agonist)<\/li>\n<\/ul>\n<p>Side effects of GLP-1 agonists include weight loss and nausea (<a href=\"http:\/\/www.jci.org\/articles\/view\/68295\">R<\/a>).<\/p>\n<p>GLP-1 agonists have been associated with asymptomatic pancreatitis and pancreatic cancer in limited studies, but this association is still being studied (<a href=\"http:\/\/care.diabetesjournals.org\/content\/33\/2\/453.full\">R<\/a>).<\/p>\n<h3><span id=\"DPP-4_inhibitors\"><a id=\"post-19089-_k8hccjs5scxd\"><\/a>DPP-4 inhibitors<\/span><\/h3>\n<p>DPP-4 inhibitors are another class of anti-diabetic drugs. They reduce the activity of an enzyme called DPP-4, which <em>deactivates<\/em> GLP-1 and another incretin called insulinotropic polypeptide (GIP). Both GLP-1 and GIP are released after meals to reduce blood sugar (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3410278\/\">R<\/a>).<\/p>\n<p>Thus, these drugs support glucose balance and GLP-1 activity in the body. DPP-4 inhibitors include:<\/p>\n<ul>\n<li>Sitagliptin (Januvia) (<a href=\"http:\/\/www.januvia.com\/sitagliptin\/januvia\/consumer\/about-januvia\/index.xhtml\">R<\/a>)<\/li>\n<li>Saxagliptin (Onglyza) (<a href=\"https:\/\/www.onglyza.com\/\">R<\/a>)<\/li>\n<li>Linagliptin (Tradjenta) (<a href=\"https:\/\/www.tradjenta.com\/\">R<\/a>)<\/li>\n<\/ul>\n<p>If you have diabetes, your doctor will discuss your medication options with you in detail, including the potential use of GLP-1 agonists or DPP-4 inhibitors.<\/p>\n<h2><span id=\"GLP-1_Research_Proposed_Health_Effects\"><a id=\"post-19089-_2tneq8p3mqb9\"><\/a><strong>GLP-1 Research &amp; Proposed Health Effects<\/strong><\/span><\/h2>\n<h3><span id=\"Blood_Glucose_Diabetes\"><a id=\"post-19089-_gty3wd7akrn5\"><\/a><strong>Blood Glucose &amp; Diabetes<\/strong><\/span><\/h3>\n<p>As mentioned, substances that act similar to GLP agonists &#8211; so-called GLP-1 agonists &#8211; are used as anti-diabetic medications.<\/p>\n<p>GLP-1 increases <a href=\"https:\/\/selfhacked.com\/blog\/insulin-101\/\">insulin<\/a> and decreases<a href=\"https:\/\/selfhacked.com\/blog\/glucagon-levels\/\"> glucagon<\/a> levels, which will result in decreased blood glucose (<a href=\"https:\/\/courses.washington.edu\/conj\/bess\/incretins\/incretins.html\">R<\/a>, <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20816021\">R<\/a>).<\/p>\n<p>GLP-1 seems to delay stomach emptying, which slows down<a href=\"https:\/\/selfhacked.com\/blog\/glucose\/?utm_source=seo&amp;utm_medium=selfhacked&amp;utm_campaign=id00002\"> glucose<\/a> spikes and makes it easier for the body to maintain blood sugar levels right after a meal (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17498508\">R<\/a>).<\/p>\n<p>In animals with diabetes, GLP-1 agonists improve the function of insulin-producing beta cells in the pancreas. Scientists think that these drugs have the potential to regenerate pancreatic cells, but this hasn\u2019t been proven in humans (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22158421\">R<\/a>,<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17164779\"> R<\/a>).<\/p>\n<p>Cell-based research is investigating whether fat muscle cells also increase glucose uptake when GLP-1 is higher (<a href=\"http:\/\/www.nature.com\/nrendo\/journal\/v8\/n12\/fig_tab\/nrendo.2012.140_F1.html\">R<\/a>).<\/p>\n<p>Before GLP-1 agonists were discovered, scientists explored the effects of unchanged GLP-1 injections in diabetics (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15655719\">R<\/a>).<\/p>\n<p>In one study, 6-weeks long administration of GLP-1 to type 2 diabetic patients reduced fasting blood glucose and hemoglobin A1c, a measure of long-term sugar control (<a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(02)07952-7\/abstract\">R<\/a>).<\/p>\n<p>However, GLP-1 has a very short half-life of about 1.5 minutes and is rapidly suppressed by an enzyme called dipeptidyl peptidase-4 (DPP-4). They suggested that GLP-1 reduces the excitement of eating food&#8211;called the<em> anticipatory food reward<\/em>, which may result in a decrease in appetite and <a href=\"http:\/\/selfhacked.com\/blog\/21-drugs-increase-motivation\/\">motivation<\/a> toward food (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26094857\">R<\/a>).<\/p>\n<p><a href=\"https:\/\/selfhacked.com\/blog\/27-hormones-involved-in-weight-lossweight-gain-not-big-4\/\">GLP-1 in mentioned in our post about hormones implicated in weight control<\/a>.<\/p>\n<p>We can\u2019t apply animal findings to humans, so we have yet to see if this hormone really reduces appetite and weight in people who struggle with obesity.<\/p>\n<p>Many substances that are initially claimed to cause weight loss in animals fail further clinical research.<\/p>\n<p>Let\u2019s remember <a href=\"https:\/\/selfhacked.com\/blog\/all-about-leptin-its-role-in-chronic-inflammation-cfs-and-weight\/\">leptin<\/a> was initially called an \u201cobesity wonder drug\u201d based on animal experiments. Well, it turned out that leptin actually better fits the nickname \u201cfat hormone\u201d and we\u2019re still in the dark as to what it exactly does in the human body. GLP-1 obesity research might face a similar fate.<\/p>\n<h3><span id=\"Brain_Mental_Health\"><a id=\"post-19089-_sarnrutilk2m\"><\/a><strong>Brain &amp; Mental Health<\/strong><\/span><\/h3>\n<p>A study on rats suggested that GLP-1R in the hippocampus enhanced learning and memory. According to one unproven hypothesis, GLP-1 might be involved in protecting the brain from seizures and neuronal damage. If proven true, this would add to our knowledge about the gut-brain connection (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12925848?dopt=Abstract\">R<\/a>).<\/p>\n<p>A GLP-1 agonist and anti-diabetic drug, exenatide, is also being researched in neurogenerative disorders. Given to 45 patients with Parkinson\u2019s disease, exenatide seemed to improve motor and cognitive symptoms compared to the control. Side effects were weight loss and nausea. Large-scale studies are needed (<a href=\"http:\/\/www.jci.org\/articles\/view\/68295\">R<\/a>).<\/p>\n<p>Some scientists have proposed that the potentially protective properties of GLP-1 agonists should also be researched in Alzheimer\u2019s disease (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/25418147\">R<\/a>).<\/p>\n<p>In lab animals, GLP-1 seems to improve mood and anxiety. <a href=\"https:\/\/selfhacked.com\/blog\/how-to-increase-orexin-and-decrease-fatigue-naturally\/\">It is hypothesized to activate <\/a><a href=\"http:\/\/selfhacked.com\/blog\/how-to-increase-orexin-and-decrease-fatigue-naturally\/\">orexin<\/a><a href=\"https:\/\/selfhacked.com\/blog\/how-to-increase-orexin-and-decrease-fatigue-naturally\/\">, which increases wakefulness<\/a>. However, the impact of GLP-1 on mood and wakefulness in humans is unknown (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26724568\">R<\/a>).<\/p>\n<h3><span id=\"Other_Research_Areas\"><a id=\"post-19089-_4ftl3zo9329n\"><\/a><strong>Other Research Areas <\/strong><\/span><\/h3>\n<p>Scientists are investigating whether GLP-1 supports heart health (<a href=\"http:\/\/www.nature.com\/nrendo\/journal\/v8\/n12\/fig_tab\/nrendo.2012.140_F1.html\">R<\/a>), relaxes blood vessels (vasodilation) (<a href=\"http:\/\/www.nature.com\/nrendo\/journal\/v8\/n12\/fig_tab\/nrendo.2012.140_F1.html\">R<\/a>), and increases the excretion of<a href=\"https:\/\/selfhacked.com\/blog\/is-salt-good-or-bad-the-science-on-the-health-effects-of-salt\/\"> sodium<\/a> (<a href=\"http:\/\/www.nature.com\/nrendo\/journal\/v8\/n12\/fig_tab\/nrendo.2012.140_F1.html\">R<\/a>)<\/p>\n<p>GLP-1 may also be involved in some potentially detrimental pathways. In some animal studies, GLP-1 appeared to increase the <a href=\"https:\/\/selfhacked.com\/blog\/factors-counteract-stress-response\/\">stress response<\/a> and <a href=\"https:\/\/selfhacked.com\/blog\/natural-treatments-anxiety\/\">anxiety<\/a> (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12867498\">R<\/a>, <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26724568\">R<\/a>).<\/p>\n<p>Another animal experiment suggested that GLP-1 and GLP-1R may be responsible for increased blood pressure and <a href=\"https:\/\/selfhacked.com\/blog\/resting-heart-rate\/\">heart rate<\/a> in response to stress.
